The project builds on services in the home environment from previous projects and programs by applying flexible mechanisms for combined hourly use of services, oriented to the personal needs of users. Quality services are planned and managed for personal care and health care, utilities and services for social inclusion and social support. By advising, motivational and psychological support, implementation of individual service plans and individual developing programs, through the provision of integrated services by a multidisciplinary team, we move towards providing long-term care and creating opportunities for the disadvantaged person to control the environment and manage his/her own life. Representatives of the two target groups have access to long-term social and health care by providing integrated services according to the specific needs of disadvantaged people. The project includes people with severe socio-health problems from the two target groups — People with permanent disabilities and their families and people over 65 with restrictions or inability to self-service. The project creates new jobs with 51 persons re-entering the labour market; the professional qualities of local human resources are increasing through training and supervision. Проектът надгражда услугите в домашна среда от предходни проекти и програми чрез прилагане на гъвкави механизми за съчетано почасово ползване на услуги, ориентирани към личните нужди на потребителите. Планират се и управляват качествени услуги за лична помощ и здравни грижи, помощ при комунално - битови дейности и услуги за социално включване и социална подкрепа. Чрез консултиране, мотивационна и психологическа подкрепа, изпълнение на индивидуални планове за услуги и индивидуални развиващи програми, чрез предоставянето на интегрирани услуги от мултидисциплинарен екип се върви по посока осигуряване на дългосрочни грижи и създаване на възможности за човека в неравностойно положение сам да контролира средата и да управлява живота си. Представителите на двете целеви групи получават достъп до дългосрочна социално-здравна грижа чрез предоставяне на интегрирани услуги, според специфичните потребности на хората в неравностойно положение. В проекта са включени лица с тежки социално-здравни проблеми от двете целеви групи – Хора с трайни увреждания и техните семейства и хора над 65 г. с ограничения или невъзможност за самообслужване. Чрез проекта се създават нови работни места, като 51 лица се включват отново в пазара на труда; увеличават се професионалните качества на местния човешки ресурс чрез обучения и супервизии.
