Celem głównym proj jest dokonanie postępu w procesie aktywizacji społeczno- zatrudnieniowej przez min. 34% spośród 58 osób (34K, 24 M), zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących w myśl KC woj. wielkopolskie, Gminę Białośliwie, poprzez udział w kompleksowym wsparciu w okresie od 01.06.2018 do 30.11.2019.W ramach projektu założono osiągnięcie następujących wskaźników:• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie – 58• Liczba osób z niepełnosprawnościami objętych wsparciem w programie - 15• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu – 18• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu – 20.• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) – 12. Grupę docelową stanowi 58 osób (34 K, 24 M) zamieszkałych w Gminie Białośliwie, zagrożonych ubóstwem lub wykluczeniem społ. Do proj zostaną przyjęte osoby spełniające min. 1 przesłankę zagrożenia ubóstwem lub wykluczeniem społecznym:- os bierne ZAW - 58 UP (34 K, 34 M),- os niepełnosprawne – 15 UP, w tym 10 o stopniu umiarkowanym- os korzystające z POPŻ - 18 os (9K, 9 M) - min. 30% UP.W ramach projektu zostaną zrealizowane takie zadania, jak:1. Aktywizacja społeczna2. Aktywizacja zdrowotna3. Aktywizacja edukacyjna4. Aktywizacja zawodowa5. Działania o charakterze środowiskowym. (Polish)

The main objective of the project is to make progress in the process of social and employment activation by at least 34 % of the 58 people (34K, 24 M), at risk of poverty or social exclusion, living in accordance with the Wielkopolska Voivodeship Voivodeship, the Białośliwie Municipality, through participation in comprehensive support in the period from 01.06.2018 to 30.11.2019.The project provides for the achievement of the following indicators:• Number of people at risk of poverty or social exclusion supported in the programme – 58• Number of people with disabilities – 15 % of people supported by the programme, or the number of people at risk of poverty or social exclusion supported by the programme – 58• Number of people with disabilities – 20 % of the population affected by the programme – or the number of people at risk of social exclusion – 58• Number of people with disabilities in support of the programme, or the number of people at risk of social exclusion – from the programme – 58• Number of people with disabilities – 20 % of the total of the 24 M persons at risk of poverty or social exclusion, who are at risk of poverty or social exclusion, are at risk of poverty or social exclusion.The aim of the project is to make progress in the process of social and employment activation by at least 34 % of the 58 people (34K, 24 M), at risk of poverty or social exclusion in the Wielkopolskie Voivodship of the Regions of Wielkopolskie Voivodship, in accordance with the Regional Council of the Wielkopolskie Voivodships, the Whitey Community, through participation in comprehensive support in the period 01.06.2018 to 30.11.2019.The project included the achievement of the following indicators:• The number of people at risk of poverty or social exclusion in the programme – 58• The number of persons at risk of poverty in the period from 01.06.2018 to 30.11.2019.The project included the achievement of the following indicators:• Number of people at risk of poverty or social exclusion supported in the programme – 58• Number of people with disabilities – 20 % of those with disabilities, or 12 % of those at risk of poverty – 58• The number of people at risk of poverty or social exclusion in the programme – 58• Number of persons at risk of poverty or social exclusion – 58• The number of people with disabilities (15-18) or 20 % of the population at risk of poverty or social exclusion supported by the programme – 58• Number of people with disabilities or 20 % of the population of the people at risk of poverty or social exclusion supported by the programme – 58•The number of people with disabilities (20) or 12 % of those at risk of poverty or Social Exclusion – 58• Number of people with a Disabled(20) The target group consists of 58 people (34 K, 24 M) living in the Białośliwo Municipality, at risk of poverty or social exclusion. The project will include people fulfilling at least 1 premise of the risk of poverty or social exclusion:- passive ZAW – 58 UP (34 K, 34 M),- disabled persons – 15 UP, including 10 moderate- wasps benefiting from POPŻ – 18 people (9K, 9 M) – at least 30 % of the Polish People’s Party.Within the project, tasks such as:1 will be carried out.
