The active inhabitants of the Wałecki District are a project carried out in the area of the ‘Pcecki’ district of the WPR in Walcz. Implementation period: From 01.10.2017 to 31.12.2019, the project will cover 60 persons — the residents of the District of wałki and PCBPR’s customers at Wałbrzych at risk of social exclusion due to disability or foster care. Support will be provided for: 1. Children and young people of care — from primary school age to emancipation, together with people in the environment (where justified): 18 Participants for Project (OP), including 11 K, 7M. 2. Persons with disabilities: 42 UP, including 28 K, 14M. The project involves the use of active inclusion services of a kind: social, b) educational, c) health. The project participants will be covered by a personalised reintegration pathway, including social work and tailored to the recruitment phase, a set of active inclusion instruments (e.g. socioeconomic travel, training, specialist counselling). Foster families (participants’ environment) will benefit from supporting child rights of child and youth custody. The activities will result in an improvement of the different aspects of the functioning of the participants in the social dimension, which will be at least 50 % of the total population, i.e. those at risk of poverty/exclusion and severely disabled persons, people with intellectual and intellectual disabilities and those with disabilities. Aktywni mieszkańcy Powiatu Wałeckiego - to projekt realizowany na terenie powiatu wałeckiego przez PCPR w Wałczu. Okres realizacji: 01.10.2017 r. - 31.12.2019 r. Projektem objętych zostanie 60 osób – mieszkańców powiatu wałeckiego i klientów PCPR w Wałczu zagrożonych wykluczeniem społecznym z tytułu niepełnosprawności lub przebywania w pieczy zastępczej. Wsparciem objęte zostaną: 1. Dzieci i młodzież w pieczy - począwszy od wieku szkoły podstawowej, do momentu usamodzielnienia, wraz z osobami z otoczenia (w sytuacji gdy będzie to uzasadnione): 18 Uczestników Projektu (UP), w tym:11 K, 7M. 2. Osoby niepełnosprawne: 42 UP, w tym:28 K, 14M. Projekt zakłada wykorzystanie usług aktywnej integracji o charakterze: a) społecznym, b) edukacyjnym, c) zdrowotnym. Uczestnicy projektu zostaną objęci zindywidualizowaną ścieżką reintegracji (integracji społecznej/ indywidualnym programem usamodzielniania), obejmującą pracę socjalną oraz dopasowany do zdiagnozowanych na etapie rekrutacji deficytów, zestaw instrumentów aktywnej integracji (m.in. wyjazdy socjoterapeutyczne, szkolenia, doradztwo specjalistyczne). Rodziny zastępcze (otoczenie uczestników) objęte zostanie wsparciem podnoszącym kompetencje do pracy z dziećmi w pieczy w zakresie deficytów zdiagnozowanych u dzieci i młodzieży. Efektem realizowanych działań będzie poprawa różnych aspektów funkcjonowania uczestników w wymiarze społecznym, co dotyczyć będzie co najmniej50% wszystkich UP, czyli osób zagrożonych ubóstwem/wykluczeniem oraz osób o znacznym stopniu niepełnosprawności, osób z niepełnospr. intelektualną i osób z niepełnospr. sprzężonymi.
