The purpose of the project is social and professional activation of 41 people excluded or threatened with social exclusion from the Otwock poviat in the period from 01/01/2017 to 30/06/2018. The participants of the project will be persons with disabilities - 15 people (8K, 7M) and people living in 26 foster care (12K, 14M): adult foster care and adolescents over 15 years of age in substitute custody, at risk of poverty or social exclusion from the Otwock poviat, using social assistance services PCPR Otwock. The persons with disability constitute 37 % of participants in the project Support will be based on the reintegration path created individually for each person (individual diagnosis of the participants' needs and potentials), taking into account the diagnosis of a problem system or threat of a problem situation, resources, potential, predisposition, needs. A social contract, referred to in the Act of 12 March 2004 on social assistance, will be signed with each participant. Tasks: 1. Support path for persons with disabilities. 2. Support path for people living in custody. Indicators: People who are at risk of poverty or social exclusion supported by the 41-person program, People with disabilities who are supported by the 15-person program, People who are at risk of poverty or social exclusion who have obtained qualifications after leaving the 10-person program, unemployed people or social exclusion, jobseekers after leaving the 9-person program, unemployed people or social exclusion, working after leaving the program for 5 people, Percentage of people risk of poverty or social exclusion which reached the minimum level of social and employment efficiency - 56%, Percentage of people threatened by poverty or social exclusion which reached the minimum level of employment efficiency - 22%, Percentage of people . to a significant degree of disability, persons with intellectual disability and persons with disabilities who have reached the minimum level of effective social-employment rate -46%, the percentage of persons with significant disability, persons with disability intellectual and persons with coupled handicaps that have reached the minimum effective level for (English) | |||
