To this end, important human resources (teachers, social personnel, judicial protection of youth, associations involved in certain activities) are mobilised for the overall care of each student, adapted to the situation of each student within the framework of an individualised project. Each week summary meetings are organised with the coordinator, the teachers, the PJJ educator and the social workers to set the individual goals of each student for the coming week. Moreover, on a regular basis, parents of pupils are welcomed into the relay class to present their children’s achievements and to establish a dialogue with these families, often breaking up with the school institution. Throughout the year and as early as September, the coordinator is responsible for monitoring pupils who have left the relay class in the school where they continue their education in order to encourage their return to the classical school system.     **_Principales actions (moyens proposés, livrables attendus, lien avec le plan financier) :_** Pour réaliser ce projet, l'équipe met en place différents axes de travail : \- Le référentiel de base reste le programme enseigné dans les classes de Collège (Français, Mathématiques, Anglais, Informatique, SVT, Arts Plastiques et Histoire Géographie) avec intégration des évolutions apportées par la réforme du collège pour 2016. \- Des axes transversaux ayant des objectifs pédagogiques et de socialisations (synthèse équipe, accueil suivi, liaison parent/ élèves, sports collectifs, socialisation, PJJ et partenariat avec le réseau de transports urbains de Saint-Dizier). \- Une ouverture culturelle et sportive est également proposée aux jeunes afin de les sortir de leur environnement habituel et de les ouvrir au monde qui les entoure (éducation artistique, activités sportives, axe socialisation et organisation de sorties pédagogiques à Saint-Dizier, Paris \- Un travail sur l'orientation, en particulier avec les élèves de quatrième et troisième et la possibilité d'organiser des périodes de stages en entreprise pour certains, en lien avec leur projet professionnel. Pour cela des moyens humains importants (personnels enseignants, personnels sociaux, Protection judiciaire de la jeunesse, associations intervenants pour certaines activités) sont mobilisés pour une prise en charge globale de chaque élève, adaptée à la situation de chacun dans le cadre d'un projet individualisé. Chaque semaine des réunions de synthèse sont organisées avec la coordinatrice, les enseignants, l'éducateur PJJ, les assistants sociaux pour fixer les objectifs individuels de chaque élève pour la semaine à venir. Par ailleurs, de manière régulière, les parents d'élèves sont accueillis au sein de la classe relais pour leur présenter les réalisations de leurs enfants et instaurer un dialogue avec ces familles souvent en rupture avec l'institution scolaire. Tout au long de l'année et dès le mois de septembre, la coordinatrice est chargée d'assurer le suivi des élèves ayant quitté la classe relais dans l'établissement où ils poursuivent leur scolarité afin de favoriser leur retour dans le système scolaire classique. Livrables : _Élèves participants_ : Fiches de présence des élèves : billets d'absence complété et signé par l'adulte en charge du groupe, récapitulatif mensuel signé par le chef d'établissement. _Enseignants valorisés dans le projet_ : fiche complétée et signée après chaque intervention et validée mensuellement par le chef d'établissement, état mensuel des heures supplémentaires réalisées, signé par le chef d'établissement et transmis au Rectorat pour mise en paiement, fiche de paye des enseignants faisant apparaître le indemnités d'heures supplémentaires correspondantes payées. _Prestataires extérieurs_ : fiche de présence complétée et signée lors de chaque séance par l'intervenant et validée mensuellement par le chef d'établissement, facture des interventions. _Prestation de voyage scolaire_ : fiche de présence des élèves et accompagnateurs, factures de la prestation. _**Aspect innovant du projet :**_ Des cours de matières d'enseignement générale sont dispensés après une évaluation du niveau réel de chaque élève. Un accent particulier est fait en orthographe, méthodologie, mathématiques comme nous le demande la circulaire éducation nationale. La Citoyenneté (ASEC) qui permettra aux élèves d'acquérir des connaissances de manière pratique sous forme de visites, de réalisations, de participation à des actions (exemple : projet Manger Bouger impliquant le sport, l'anglais (organisation d'un breakfast) et les mathématiques (conversions, calories,…), Un partenariat avec une association sportive permettant de développer chez chaque jeune une envie d'adhérer à une vie sociale et scolaire. **_Modalités de mise en oeuvre :_** Une coordinatrice à temps complet, Professeur Contractuel de lycée professionnel valorisée dans le projet pour les 14 mois d'accueil des élèves de septembre 2019 à juin 2021 _Enseignement_ : Septembre 2019 à juin 2021, 2 Professeurs de Collège ( Anglais, Mathématiques). 1 Professeur de Lycée Professionnel (Histoire Géographie).
