The project aims to implement a well functioning support model for families facing educational, social, life, risk reduction or loss of custody rights. The Foundation will provide the above 50 families with comprehensive support for the acquisition or restoration of a coherent and satisfactory life. The applicant wishes to extend the services in particular to those regions in which there is a clear deficit. In practice this means, above all, going to the small town of Opole. The applicant wishes to benefit from the knowledge and experience of 10 years of effective activity in the social assistance sector as a non-governmental partner and to disseminate best practices resulting from them. It plans to combine different forms of support and support for specific families with a wide range of actors working on the local social services market. Within the scope of the project, the family members will be covered by: Support for family assistants — specialist counselling in terms of social, psychological, pedagogical and legal training — advice on the direction of vocational/educational education for young and young adults — support for diagnosis and recovery — an important element of the project will be the provision of mediation services to settle family problems and problems arising from ongoing administrative procedures (e.g. social debts) and judicial procedures — assistance in the context of integration support groups. the Applicant will continue to support the family and children supported by the Applicant’s Support Facilities, through its Special Daily Support Facilities and those of specialists who are constantly cooperating with them. Projekt ma na celu realizację dobrze funkcjonującego modelu wsparcia dla rodzin przeżywających trudności wychowawcze, socjalne, życiowe, zagrożone ograniczeniem lub utratą praw do opieki nad dzieckiem. Fundacja dostarczy do grupy wyżej wymienionych 50 rodzin kompleksowe wsparcie mające na celu nabycie lub przywrócenie im zdolności do spójnego i satysfakcjonującego życia. Wnioskodawca chce objąć usługami zwłaszcza te rejony województwa, w których istnieje wyraźny ich deficyt. W praktyce oznacza to przede wszystkim wyjście do małych miejscowości Opolszczyzny. Wnioskodawca chce wykorzystać wiedzę i doświadczenie wynikające z 10 letniej efektywnej działalności w sektorze pomocy społecznej jako partner pozarządowy, a także upowszechnić najlepsze praktyki z nich wynikające. Planuje łączyć różne formy wsparcia a w ramach pomocy konkretnym rodzinom, współpracować z wieloma podmiotami działającymi na lokalnym rynku usług społecznych. W ramach projektu rodziny zostaną objęte: - wsparciem asystentów rodziny - doradztwem specjalistycznym z zakresu socjalnego, psychologicznego, pedagogicznego, prawnego - pomocą brokera edukacyjnego - doradztwo w zakresie kierunku kształcenia zawodowego /edukacyjnego dla młodzieży oraz młodych dorosłych - wsparciem w zakresie diagnozy i wychodzenia z uzależnień - ważnym elementem projektu będzie dostarczanie usług mediacji celem ugodowego rozwiązania problemów rodzinnych oraz problemów wynikających z toczących się postępowań administracyjnych ( np. zadłużenia czynszowe ) i sądowych - pomocą w ramach integracyjnych grup wsparcia Wnioskodawca po zakończeniu projektu nadal będzie wspierał rodziny i dzieci objęte wsparciem , poprzez prowadzone przez niego Placówki Wsparcia Dziennego oraz specjalistów stale z nim współpracujących.
