The project is aimed primarily at the elderly, including dependent people in the house as a result of the disease, from Opolskie Province. The main purpose of the project is to provide specialist medical care (nursing) in a domestic environment to an elderly person and a condition of 4 hours/day for 5 days/t in exchange for ‘carers’ and rehabilitation services (3 times a week) by 12 months. The provision of continuous care for elderly people, including independent ones, will enable the following: to cut them out of their daily care and personal care responsibilities, carers will be able to move and deal with the most pressing issues, such as: purchases, official cases, use of medical consultations, etc. They will be able to go socially and professionally, i.e. find a job. activities also foresee a workshop cycle for medical topics (e.g. first-aid, rehabilitation, psychological, nursing and other) to acquire and improve skills and knowledge of the treatment of older and non-dependent persons. In addition, the project will provide for a series of specialised medical knowledge training, which increases the knowledge and qualifications of healthcare professionals providing services to the elderly, including independent care. this will enable the creation of sustainable/developed solutions to improve the quality of the health service provided in the region — to develop a new comprehensive interdisciplinary behaviour with a person with a disability through the creation of a multidisciplinary team of senior/dependent services and comprehensive coverage by accessibility to the rehabilitation pages. duration of the project 01.10.2016-30.11.2017. Projekt skierowany jest przede wszystkim do osób starszych, w tym niesamodzielnych pozostających w domu w wyniku choroby, z terenu województwa opolskiego. Celem głównym projektu jest zapewnienie specjalistycznej opieki medycznej (usług opiekuńczo –pielęgniarskich)w warunkach domowych osobie starszej i schorowanej w wymiarze 4 h/dziennie przez 5 dni/tyg. w zamian za opiekunów faktycznych oraz usług rehabilitacyjnych (3 razy w tygodniu) przez 12 m-cy. Zapewnienie stałej opieki nad osobą starszą, w tym niesamodzielną pozwoli opiekunom faktycznym.: na odciążenie ich z codziennych obowiązków opiekuńczych i pielęgnacyjnych, opiekunowie będą mogli wyjść i spokojnie załatwić najpotrzebniejsze sprawy jak: zakupy, sprawy urzędowe, skorzystać z konsultacji lekarskiej itp. Będą mogli realizować się społecznie i zawodowo tzn. znaleźć zatrudnienie.W ramach działań przewiduje się również realizację cyklu warsztatów dla op. faktycznych z zakresu tematyki medycznej (np. pierwsza pomoc, opieka rehabilitacyjna, psychologiczna, pielęgniarska i inne) w celu nabycia oraz doskonalenia umiejętności i wiedzy dotyczącej podejścia do opieki nad osobą starszą i niesamodzielną. Dodatkowo projekt zakłada przeprowadzenie cyklu specjalistycznych szkoleń z zakresu wiedzy medycznej, podnoszących wiedzę i kwalifikacje personelu medycznego świadczącego usługi na rzecz osób starszych, w tym niesamodzielnych.Umożliwi to stworzenia trwałych/wypracowanych rozwiązań podnoszenia jakości u. świadczonych w regionie – wypracowanie nowego kompleksowego interdyscyplinarnego postępowania z osobą chorą poprzez stworzenie interdyscyplinarnego zespołu spec. świadczących usługi na rzecz os. starszych/niesamodzielnych oraz kompleksowego zabezpieczenia chorego poprzez dostępność do s. rehabilitacyjnego.Czas trwania projektu 01.10.2016- 30.11.2017.
