The main objective of the project is to increase access to social services for 60. (46k, 14M) at risk of poverty or social exclusion as a target group, including 10. (8k, 2M) with disabilities, living in Gm. For the implementation of the 2 tasks: 1. The establishment of a daily form of care services in the form of a Senior Citizens’ Club will be carried out in the framework of this task. 2. The day-to-day running of the Seniora Club. The following are planned for project participants: (a) activities aimed at providing healthcare, anti-loneliness actions and developing individual interests and activities. It will take 798h (b) a swimming pool, a cinema theatre and theatre. It will be implemented. 25 exits. (c) inclusive, dance and music evenings, and canokayal tides. In order to address needs in support of dependent persons, elderly and disabled persons from the municipality of Nowa Sear. Indicators: (a) the product: — Number of persons involved in training/advice in the field of digital competency (20). — Number of people at risk of poverty or social exclusion covered by social services of general interest in the -60 programme. — at-risk-of-poverty or social exclusion of social assistance services provided by the local community in the programme -60. — Number of actors using information and communication technologies (ICT) -1. b) results: — Number of social service sites supported in the programme after the completion of the proj-60. — Number of places in the provision of assistant services and care services existing at the end of the provision of personal assistance (60). Celem głównym proj. jest zwiększenie dostępu do usług społecznych dla 60 os. (46K,14M) zagrożonych ubóstwem lub wykluczeniem społ., stanowiących grupę docelową proj., w tym 10 os. (8K,2M) z niepełnosprawnościami, zamieszkujących Gm. Nowa Sucha (woj. mazowieckie), poprzez utworzenie i zapewnienie bieżącego funkcjonowania Klubu Seniora (miejsca świadczeń usług opiekuńczych) w Nowej Suchej w okresie od 1.11.2018 do 31.10.2020. W proj. zaplan. realizację 2 zadań: 1. Utworzenie dziennej formy usług opiekuńczych w postaci Klubu Seniora W ramach zadania zostaną dostos. i doposażone pomieszczenia w budynku GOKSiR w Nowej Suchej, w którym będzie funkcj. nowo utworzony Klub Seniora. 2. Bieżące funkcjonowanie Klubu Seniora. Dla uczestników projektów zaplanowano: a) zajęcia mające na celu działalność prozdrowotną, przeciwdziałaniu osamotnieniu oraz rozwijające indywidualne zainteresowania i aktywność. Zostanie zrealizowanych 798h zajęć. b) wyjazdy na basen, do kina i teatru. Zostanie zrealiz. 25 wyjazdów. c) festyny integracyjne, wieczory taneczno-muzyczne oraz spływy kajakowe. Proj. realizowany jest w odpowiedzi na zdiagn. potrzeby w zakresie wsparcia osób niesamodzielnych, starszych i z niepełnospr. z terenu Gminy Nowa Sucha. Wskaźniki: a) produktu: -Liczba osób objętych szkoleniami/doradztwem w zakresie kompetencji cyfrowych-20. -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie-60. -Liczba osób zagroż. ubóstwem lub wykl. społecznym objętych usługami asystenckimi i opiekuńczymi świadczonymi w społeczności lokalnej w programie -60. -Liczba podmiotów wykorzystujących technologie inform.-komunikacyjne (TIK)-1. b) rezultatu: -Liczba wspartych w programie miejsc świadczenia usług społ., istniejących po zakończeniu proj.-60. -Liczba utw. w progr. miejsc świadczenia usług asystenckich i opiek. istniejących po zakoń. proj.-60.
