The project “Cooperation — Integration-Help” is aimed at 80 inhabitants of the municipality of Legionella, who are disabled and/or dependent persons, and the factual carers of the group. On the basis of the diagnosis made, the greatest need is within the group of adult ON. This group is no longer covered by the educational system and the children are losing their chances of working in society as they become increasingly vulnerable to social exclusion. Therefore, the project is a comprehensive support supporting the Group’s list of diagnosed activities. The applicant envisages the implementation of individual support pathways, a support group for factual carers, specialised guidance, legal advice, workshops. The key to the project is the creation of a place of care, the place where care services are provided in the form of short-term daily stays, which will primarily serve to improve access to care services in place for carers in the framework of real care, and increase the number of social service provision sites in the municipality of Legionowo. The applicant will also open a rehabilitation equipment for rehabilitation equipment. The project “Co-Operation-Integration-Assistance” will be implemented in a multi-sectoral partnership with a local NGO for the benefit of persons with disabilities. (English)

    Projekt „Współpraca-Integracja-Pomoc” skierowany jest do 80 mieszkańców gminy Legionowo, którzy są osobami niepełnosprawnymi lub/i niesamodzielnymi oraz do opiekunów faktycznych powyższej grupy. Z przeprowadzonej diagnozy wynika, że największe potrzeby są w grupie os. zajmujących się dorosłymi ON. Grupa ta bowiem nie jest już objęta systemem edukacyjnym a dzieci stając się pełnoletnie tracą możliwości na funkcjonowanie w społeczeństwie stając się coraz bardziej narażone na wykluczenie społeczne. Dlatego projekt jest kompleksowym wsparciem odpowiadającym na potrzeby grupy, zawierającym katalog zdiagnozowanych działań. Wnioskodawca przewiduje realizację ścieżek wsparcia indywidualnego, grupę wsparcia dla opiekunów faktycznych, poradnictwo specjalistyczne, poradnictwo prawne, warsztaty. Kluczowe dla projektu jest utworzenie miejsca opieki wytchnieniowej – miejsca świadczenia usług opiekuńczych w formie krótkookresowego pobytu dziennego, które posłuży przede wszystkim poprawie dostępu do usług opiekuńczych w zastępstwie za opiekunów faktycznych w ramach opieki wytchnieniowej oraz zwiększa liczbę miejsc świadczenia usług społecznych na terenie Gminy Miejskiej Legionowo. Wnioskodawca uruchomi również wypożyczalnię sprzętu rehabilitacyjnego. Projekt „Współpraca-Integracja-Pomoc” realizowany będzie w partnerstwie wielosektorowym z lokalną organizacją pozarządową działającą na rzecz osób niepełnosprawnych. (Polish)
