Celem głównym projektu jest rozwój i upowszechnianie form wsparcia środowiskowego poprzez tworzenie warunków do opieki domowej z funkcją wypożyczalni sprzętu pielęgnacyjnego, rehabilitacyjnego i wspomagającego w miejscu zamieszkania dla 50 osób niesamodzielnych z terenu Miasta Zamość w okresie do 30.09.2020r. W ramach projektu utworzymy biuro wsparcia osób niesamodzielnych z wypożyczalnią sprzatu pielęgnacyjnego, rehabilitacyjnego i wspomagającego, gdzie obejmiemy wsparciem usługami opiekuńczymi i specjalistycznymi usługami opiekuńczymi 50 osób w miejscu zamieszkania z terenu Miasta Zamość. W wyniku realizacji projektu powstanie 50 miejsc opieki domowej nad osobami niesamodzielnymi dostosowane do potrzeb i oczekiwań opiemkunów i uczestników projektu. Główne zadania projektowe to: - adaptacja pomieszczeń na biuro i pomieszczenia wypożyczalni sprzętu rehabilitacyjnego, pielęgnacyjnego i wspomagającego wraz z otoczeniem oraz zakup wyposażenia na potrzeby projektu - Rekrutacja uczestników do projektu - Świadczenie usług opiekuńczych (dalej UO) - Świadczenie specjalistycznych usług opiekuńczych i zdrowotnych (SUOiZ) (Polish)
