Le diagnostic partagé par l'ensemble des acteurs de l'insertion professionnelle est que la réussite d'une prise en charge passe par un parcours fluide, qui évite de trop longues ruptures. Conscient qu'il s'agit d'un enjeu majeur, le Service Emploi Formation de l'ARS, acteur reconnu sur le territoire Terres de Lorraine, se positionne pour la réalisation d'une prestation de coordination des parcours telle que définie dans le cahier des charges de l'action : * **Finalité :** Éviter toute rupture dans le parcours des participants. * **Missions du référent prestataire :** * **1ère mission : Coordonner et assurer un suivi personnalisé (entre 65 et 80 participants).** Les participants seront orientés vers le dispositif par : * Pôle Emploi * Les structures d'insertion par l'activité économique * Les services d'action sociale des communes * Les services du Conseil Général * Les espaces emploi * Les opérateurs conventionnés par le Conseil Général ou par le PLIE. Entrée dans le dispositif : Avant l'intégration sur l'action, le référent veillera à ce que le participant ne soit pas déjà accompagné par une autre structure afin d'éviter les doublons ou d'envisager un passage de relais si celui-ci parait plus pertinent. Le prestataire sera aussi attentif aux critères d'entrées administratifs (B RSA, DELD, DE de plus de 45 ans, femmes sans qualification professionnelle, chefs de famille mono parentales, personnes ayant une Reconnaissance de la Qualité de Travailleur Handicapé). Mais il veillera aussi à ce que la personne soit prête à s'investir dans cet accompagnement et qu'il souhaite avancer dans son parcours d'insertion. Lors du premier entretien avec le prescripteur, il s'agira de vérifier cette adhésion. A l'issue du premier entretien, un échange pourra être réalisé entre le prescripteur, le Conseil Général, la MEEF et le Pole Emploi afin d'envisager ou non l'entrée sur le dispositif. Toute entrée sur l'action ou annulation de commande se fera en accord avec le PLIE et le SESIT TDL. Déroulement de l'accompagnement : Le SEF préconise que le premier entretien se fasse avec le prescripteur afin d'échanger sur la situation et assurer une cohérence dans le parcours pour le participant. Le conseiller proposera donc systématiquement cette option au prescripteur. Si ce premier entretien tripartite n'est pas possible, le prestataire prendra contact avec le prescripteur afin d'avoir un échange par mail ou par téléphone. **A/Présentation de l'accompagnement proposé et assurance de l'adhésion de la personne.** Lors du premier entretien il s'agit de présenter à la personne l'accompagnement qui lui est proposé en s'assurant qu'elle soit disposée à s'investir dans le cheminement de son parcours d'insertion. Elle sera alors informée du fait que cet accompagnement est cofinancé par le Conseil Général et le PLIE sur des Fonds Sociaux Européens qu'il fait donc partie de son parcours au sein du PLIE. Par le biais de la signature de la fiche étape PLIE, le conseiller expliquera à la personne les engagements qu'elle prend. **B/Diagnostic approfondi.** Les rendez vous suivants ont pour objectif d'établir **un diagnostic complet sur la situation socio professionnelle de la personne** par le biais de la fiche d'accueil élaborée par le SEF. Cette fiche est un support administratif qui permet au référent de recueillir les éléments qu'il approfondira ensuite avec la personne en la questionnant sur différents points (parcours professionnel, éventuels problèmes de santé, mode de garde des enfants, situation par rapport au logement, (French)
