The main aim of the project is to improve the life situation of 15 people living in the municipality of Łubianka by increasing the availability of quality care services in the local community and increasing the skills of 12 carers for dependants by 31.10.2019. The project is aimed at the residents of the municipality of Łubianka, including 15 persons (9K, 6M) who are at risk of poverty or social exclusion and 12 carers of fact (9K, 3M) of dependent persons (i.e. those who are living at risk of poverty or social exclusion). The main tasks in the project (in addition to recruitment) will be (1) the creation of 15 residential care services provided in the daily home of Domie Pytu in the town of Bierwłowo, which consists of adapting the building for the operation of the DDP and purchasing the necessary equipment for the functioning of the DDP, in accordance with the performance standards of this type of service, (2) Conduct of the DDP — support for dependent persons through the provision of care services in a comprehensive and individualised manner to the needs of the participants, (3) Maintenance of the DDP — support for carers of the fact and improving care services for dependants. The following deliverables and outputs will be achieved in connection with the implementation of the project: 1. The number of people at risk of poverty or social exclusion covered by the general interest social services in the programme — 152. Number of social services existing at the end of the project supported in the programme — 153. Number of factual carers, people in the surrounding area of non-dependent persons included in the project — 124. Celem głównym projektu jest poprawa sytuacji życiowej 15 osób niesamodzielnych z terenu Gminy Łubianka poprzez zwiększenie dostępności do wysokiej jakości usług opiekuńczych świadczonych w społeczności lokalnej i wzrost umiejętności 12 opiekunów faktycznych w zakresie opieki nad osobami niesamodzielnymi do 31.10.2019 roku. Projekt adresowany jest do osób zamieszkujących zgodnie z KC Gminę Łubianka, w tym 15 osób (9K, 6M)niesamodzielnych zagrożonych ubóstwem lub wykluczeniem społecznym oraz 12 opiekunów faktycznych (9K, 3M) osób niesamodzielnych (tj. osób z otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym). Głównymi zadaniami w projekcie (prócz rekrutacji) będą:1) Utworzenie 15 miejsc usług opiekuńczych świadczonych w Dziennym Domie Pobytu w miejscowości Bierzgłowo, gm. Łubianka, polegające na adaptacji budynku na potrzeby funkcjonowania DDP oraz zakupie niezbędnego wyposażenia dla funkcjonowania DDP, zgodnie ze standardami funkcjonowania tego typu placówki,2) Prowadzenie DDP - wsparcie dla osób niesamodzielnych poprzez świadczenie usług opiekuńczych dobranych w sposób kompleksowy i zindywidualizowany do potrzeb uczestników,3) Prowadzenie DDP - wsparcie dla opiekunów faktycznych w zakresie pełnienie usług opiekuńczych i zwiększenia ich umiejętności w zakresie opieki nad osobami niesamodzielnymi. W związku z realizacja projektu zostaną osiągnięte następujące rezultaty i produkty:1. Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie - 152. Liczba wspartych w programie miejsc świadczenia usług społecznych istniejących po zakończeniu projektu - 153. Liczna opiekunów faktycznych, osób z otoczenia osób niesamodzielnych objętych wsparcie w projekcie - 124. Liczba opiekunów faktycznych, osób z otoczenia osób niesamodzielnych u których nastąpił przyrost w zakresie umiejętności opiekuńczych nad osobami niesamodzielnymi - 10. (Polish)
