The project aims at setting up an Environmental Services Centre to improve access to care and assistance services for 50 persons (30K and 20M) at risk of poverty and social exclusion and to reside there. (28 K and 12M) provided in the local community, as well as improved access to services supporting the social activation process from 30.08.2019 to 29.08.2021. 1) Improving access to unaccompanied care services for dependent persons who require care or support because of the inability of themselves to carry out at least one of the everyday activities of the local community, including the development of non-fixed care services, including: care services and specialist services at home and assistance services tailored to individual needs. 2) Provision of support services for the social activation process including: legal advice on family and carers’ law, social security, protection of tenants’ rights and psychological counselling — forms of support to the ON, (c) Group support for the improvement of life competences and skills to overcome people living in the environment of people at risk of poverty or social exclusion, so-called “factice carers”, (d) advisory services in crisis situations. Warned. RESULTS: 30 persons (17K and 13M) will gain access to assistant and/or care services, 40 people in the environment (28K and 12M) will increase the level of care responsibilities. Projekt ma na celu utworzenie Centrum Usług Środowiskowych w celu poprawy dostępu do usług opiekuńczych i asystenckich dla 50 osób (30K i 20M) zagrożonych ubóstwem i wykluczeniem społ., zamiesz. w rozumieniu kod. cywilnego pracujących/uczących się w powiecie aleksandrowskim z gmin Raciążek, Ciechocinek, Aleksandrów Kujawski oraz 40 osób z otoczenia osób zagrożonych ubóstwem lub wykluczeniem społ. (28 K i 12M) świadczonych w lokalnej społeczności, a także poprawa dostępu do usług wspierających proces aktywizacji społecznozawodowej od 30.08.2019 do 29.08.2021 r. Realizowane ZADANIA: 1) Poprawa dostępu do usł. opiekuńczych/asystenckich dla osób niesamodzielnych, które wymagają opieki lub wsparcia w związku z niemożnością samodzielnego wykonywania co najmniej jednej z podst. czynności życia codziennego, świadczone w lokalnej społ. obejmujące rozwój niestacjonarnych usł. opiekuńczych, w tym: usług opiekuńczych i specjalistycznych usług opiekuńczych w miejscu zamieszkania oraz usług asystenckich dopasowanych do indywidualnych potrzeb. 2) Zapewnienie usług wspierających proces aktywizacji społ-zaw obejmujące: poradnictwo prawne w zakresie prawa rodzinnego i opiekuńczego, zabezpieczenia społecznego, ochrony praw lokatorów oraz poradnictwo psychologiczne - formy wsparcia dla ON, c) Wsparcie grupowe w zakresie podniesienia kompetencji życiowych i umiejętności zaw., służących przezwyciężeniu nieporadności życiowej osób z otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym, tzw. opiekunowie faktyczni, d) doradztwo indywid. w sytuacjach kryzysowych. Gł. REZULTATY: 30 osób (17K i 13M) uzyska dostęp do usług asystenckich i/lub opiekuńczych, 40 osób z otoczenia (28K i 12M) podniesie poziom kompetencji z zakresu opieki.
