The objective is to provide 36 dependants living in the municipality of Gniewkowo with easier access to affordable, durable and high-quality services of general interest over a period of 24 months, by increasing the number of care facilities, by creating 8 new residential care services (8 carers). This target will be achieved in the period 01.09.2017-31.08.2019. The support covers persons who are resident within the meaning of the CC in the municipality of Gniewkowo, in the area of Inowrocław, Kujawsko-Pomorskie Province. This will be achieved by: 1. The organisation of an environmental service centre (CUŚ) in terms of services supporting the activation of the social and professional activation process, which will operate for participants in the community environment for the target group and for carers of genuine dependent persons, and will involve local actors and institutions (including in particular PES, mainly from the district of Inowrocław). 2. Provision of care services and specialist care services in the place of residence of dependent persons. 3. Supporting carers of genuine dependent persons. The project is addressed directly to the following groups of recipients: 1. People at risk of poverty or social exclusion. 2. The environment of people at risk of poverty or social exclusion (to the extent necessary to support socially excluded people), including those with caring responsibilities. Result: 1. Number of social service sites supported in the scheme, supported by the programme: 8 carers; 2. The number of non-dependent persons covered by the care services -36 3. Celem proj. jest zapewnienie 36 osobom niesamodzielnym zamieszkującym Gminę Gniewkowo w okresie 24 miesięcy realizacji proj. łatwiejszego dostępu do przystępnych cenowo, trwałych oraz wysokiej jakości usług, świadczonych w interesie ogólnym, poprzez zwiększenie liczby miejsc świadczenia usług opiekuńczych, poprzez utworzenie 8 nowych miejsc świadczenia niestacjonarnych usług opiekuńczych (8 opiekunów/ek). Powyższy cel zrealizowany zostanie w okresie 01.09.2017r.-31.08.2019r. Wsparcie obejmuje osoby zamieszkujące w rozumieniu KC na terenie gminy Gniewkowo, w pow. inowrocławskim, w woj. kujawsko-pomorskim. Powyższy cel zrealizowany zostanie poprzez: 1. Organizację centrum usług środowiskowych (CUŚ) w zakresie usług wspierających proces aktywizacji społecznej i zawodowej, który działać będzie dla uczestników proj. dla otoczenia grupy docelowej oraz dla opiekunów faktycznych osób niesamodzielnych i będzie angażował do współpracy lokalne podmioty i instytucje (w tym w szczeg. działające PES, przede wszystkim z terenu powiatu inowrocławskiego). 2. Świadczenia usług opiekuńczych i specjalistycznych usług opiekuńczych w miejscu zamieszkania osób niesamodzielnych. 3. Wspieranie opiekunów faktycznych osób niesamodzielnych. Projekt skierowany jest bezpośrednio do poniższej grup odbiorców: 1. Osoby zagrożone ubóstwem lub wykluczeniem społecznym. 2. Otoczenie osób zagrożonych ubóstwem lub wykluczeniem społecznym (w takim zakresie, w jakim jest to niezbędne dla wsparcia osób wykluczonych społecznie), w tym osoby pełniące obowiązki opiekuńcze. Rezultat: 1. Liczba wspartych w programie miejsc świadczenia usług społ. istniejących po zakończeniu proj: 8 opiekunów/ek; 2. Liczba osób niesamodzielnych objętych usługami opiekuńczymi -36 3. Liczba opiekunów faktycznych objęta wsparciem w projekcie -16 osób. (Polish)
