The project will improve access to specialist care services at home for disabled people on grounds of disability or illness, in particular for children and adolescents by 30 for physiotherapy/rehabilitation (30 people). The project also covers the person’s environment in terms of psychological and nutritional support (30 persons). the project aims at the prevention of social exclusion, isolation from the family and the local community through individualised support. The specialised services will be provided at the place of residence of the beneficiary (access to the disabled person). The service provided by the physiotherapist and rehabilitation will be provided at the place of residence. (disability, illness) and 30 persons in their surroundings (to the extent necessary to support them). As a result, 2 places of supply of the social services existing after the end of the project will be supported. Physical rehabilitation and improvement of disturbed function of the body occurs to an extent not covered by the Act of 27 August 2004 on healthcare services financed from public funds Journal of Laws No 210, item 2135, as amended), in accordance with a medical recommendation or specialist in the field of motor rehabilitation or physiotherapy.     Projekt polegać będzie na poprawie dostępu do specjalistycznych usług opiekuńczych w miejscu zamieszkania dla osób niesamodzielnych ze względu na niepełnospr. lub chorobę, w szczególności dla dzieci i młodzieży do 30 r.ż.w zakresie fizjoterapii/rehabilitacji (30 os.). Projekt obejmuje także otoczenie tych osób w zakresie wsparcia psychologa i dietetyka (30 os.) (rodzic/opiekun faktyczny osoby niesam.).Projekt ma na celu zapobieganie wykluczeniu społ.,odizolowaniu od rodziny i społ. lokalnej poprzez zindywidualizowany zakres wsparcia. Usługi specjalistyczne świadczone będą w miejscu zamieszkania osoby korzystającej (dojazd do osoby niepełnospr.).Z porad psychologicznych i konsultacji dietetyka będzie można skorzystać w budynku GOPS w Rogowie,który jest przystosowany do osób z niepełnosprawnością ruchową. Usługa świadczona przez fizjoterapeutę i rehabilitanta świadczona będzie w miejscu zamieszkania.Ze wsparcia psychologa i dietetyka skorzystają osoby z otoczenia w siedzibie GOPS.Usługi,o których mowa będą świadczone w interesie ogólnym.W ramach projektu utworzone zostaną 2 miejsce świadczenia SUO w miejscu zamieszkania,z którego skorzysta 30 osób zagr. ubóstwem lub wykluczeniem społ. (niepełnospr., choroba) oraz 30 osób z otoczenia tych osób (w takim zakresie,w jakim jest to niezbędne dla wsparcia tych osób). W rezultacie zostaną wsparte 2 miejsca świadczenia usług społ. istniejące po zakończeniu projektu. Rehabilitacja fizyczna i usprawnianie zaburzonych funkcji organizmu następuje w zakresie nieobjętym przepisami ustawy z dnia 27 sierpnia 2004 r. o świadczeniach opieki zdrowotnej finansowanych ze środków publicznych (Dz. U. Nr 210, poz. 2135, z późn. zm.) zgodnie z zaleceniami lekarskimi lub specjalisty z zakresu rehabilitacji ruchowej lub fizjoterapii. (Polish)
