Assessment of the impact of the intervention by means of a quasi-experimental study before/after to determine changes in the frequency and diagnostic profitability of petitions. 5. Evaluation of the impact of adherence vs non-adherence to recommendations on significant findings in patients with clinical suspicion who are undergoing the diagnostic test. Scope: Emergencies of the Hospitals Ramón y Cajal, Cruces, La Paz, Basurto, Puerta de Hierro. Subjects: Information will be collected for the impact assessment (Sept. 2017-Sept 2019) emergency attendance, number of applications, diagnostic profitability, and data related to the use of the CDSS.     Objetivo: Establecer recomendaciones para mejorar la adecuación en la solicitud de Radiografía (Rx) y Tomografía Computarizada (TC) en el servicio de Urgencias. Análisis del impacto de la implementación de las recomendaciones. Cuantificar el impacto de no adherencia a las recomendaciones en pacientes a los que se realiza TC o Rx. Diseño: Proyecto con 5 fases: 1. Análisis sistemático y crítico de las evidencias sobre la utilidad de las TC y Rx en el servicio de urgencias. Se accederá secuencialmente a documentos de síntesis y sinopsis de evidencias, revisiones sistemáticas y estudios primarios. 2. Generación de recomendaciones mediante metodología Delphi para alcanzar un consenso en la indicación o no de un examen radiológico en las situaciones clínicas urgentes más relevantes. 3 .Implementación de la guía de recomendaciones, mediante una estrategia multifacética que incluye sistema automático de ayuda a la decisión (CDSS) integrado en el sistema de peticiones. 4. Evaluación del impacto de la intervención mediante un estudio cuasi-experimental antes/después para determinar los cambios en la frecuencia y rentabilidad diagnóstica de las peticiones. 5. Evaluación del impacto de la adherencia vs no adherencia a recomendaciones en los hallazgos significativos en aquellos pacientes con sospecha clínica a los que se realiza la prueba diagnóstica. Ámbito: Urgencias de los Hospitales Ramón y Cajal, Cruces, La Paz, Basurto, Puerta de Hierro. Sujetos: Para la evaluación del impacto se recogerá información (periodo Sept. 2017-Sept 2019) de frecuentación a urgencias, número de solicitudes, rentabilidad diagnóstica, y datos relativos a la utilización del CDSS.
