    Se pretende estudiar los cambios en la actividad del Sistema Inmune y del Sistema Nervioso Simpático (SNS) que ocurren en pacientes con cirrosis durante un seguimiento de 3 años dentro del programa de detección precoz del hepatocarcinoma (HCC) para tratar de identificar los componentes de ambos sistemas y de la interacción entre ellos asociados a la progresión de la cirrosis y el desarrollo de HCC. Para ello, se seguirá durante 3 años a un total de 200 pacientes cirróticos que cada 6 meses se someten a un control clínico, analítico y de imagen como parte del programa de detección precoz del HCC. Coincidiendo con cada evaluación de control se obtendrá a través del biobanco del Hospital General Universitario de Alicante (HGUA) una muestra sanguínea y se registrarán los datos de la situación clínica del paciente. En cada muestra sanguínea se estudiará el inmunofenotipo leucocitario diferenciando las poblaciones de células NK y linfocitos T citotóxicos y se estudiarán las características de la activación inmune midiendo las concentraciones de citoquinas pro y anti-inflamatorias (IL-12, TNFalfa, INFgamma, IL-6, IL-8, IL-10) y de citoquinas con potencial fibrogénico (TGF beta) y las concentraciones de óxido nítrico en relación a la existencia o no de patrones moleculares asociados a patógenos como DNA bacteriano y lipopolisacárido. La actividad del SNS se estudiará midiendo las concentraciones de catecolaminas circulantes, la actividad de la enzima dopamina-beta-hidroxilasa y la actividad de los receptores beta 2 adrenérgicos periféricos. Al finalizar el estudio podrá describirse la evolución y cambios tanto del sistema inmune como del SNS en relación a la existencia de estímulos inflamatorios y cambios clínicos sufridos por los pacientes que desarrollan y los que no desarrollan un HCC.     The aim is to study the changes in the activity of the Immune System and the Sympatic Nervous System (SNS) that occur in patients with cirrhosis during a 3-year follow-up within the program of early detection of hepatocarcinoma (HCC) to try to identify the components of both systems and the interaction between them associated with the progression of cirrhosis and the development of HCC. For this purpose, a total of 200 cirrhotic patients who undergo clinical, analytical and imaging monitoring every 6 months will be followed for 3 years as part of the program for the early detection of HCC. Coinciding with each control evaluation, a blood sample will be obtained through the biobank of the Hospital General Universitario de Alicante (HGUA) and data on the patient’s clinical situation will be recorded. In each blood sample, the leukocyte immunophenotype will be studied by differentiating the populations of NK cells and cytotoxic T cells and the characteristics of immune activation will be studied by measuring the concentrations of pro- and anti-inflammatory cytokines (IL-12, TNFalfa, INFgamma, IL-6, IL-8, IL-10) and cytokines with fibrogenic potential (TGF beta) and nitric oxide concentrations in relation to whether or not there are molecular patterns associated with pathogens such as bacterial DNA and lipopolysaccharide. The activity of SNS will be studied by measuring the concentrations of circulating catecholamines, the activity of the enzyme dopamine-beta-hydroxylase and the activity of peripheral adrenergic beta 2 receptors. At the end of the study, the evolution and changes of both the immune system and the SNS in relation to the existence of inflammatory stimuli and clinical changes experienced by patients who develop and those who do not develop a HCC may be described.
