Breast cancer is the most common tumor in women in the Western world. A very important fact is the change in presentation of breast tumors today. In addition, it should be mentioned that current imaging techniques have some limitations of diagnosis, over-diagnosis and a high value of false positives. It follows from this the need to look for new imaging techniques that complement the current ones and resolve these limitations. The PET functional image dedicated to breast (dbPET) is a recent technique that has provided promising results to deal with the diagnosis and follow-up of breast pathology from an alternative approach. In this project we propose to develop a study of tumor heterogeneity through multiple dbPET studies and its correlation with different clinical-biological parameters. To this end, it is proposed to consider: (I) the interest of dbPET in the diagnosis of breast lesions when this is not possible with the usual techniques; (II) the role of dbPET in cases where MRI is performed, with the aim of reducing the number of false positives of MRI; (III) the efficacy and prognostic value of dbPET in cases of neoadjuvant chemotherapy and its interest in follow-up; (IV) the radiobiology of breast brachytherapy, evident through the molecular image and its effect on the metabolism of the gland and (v) new radiopharmaceuticals that allow to define other biological facets of the tumor and that may be of clinical interest.     El cáncer de mama constituye el tumor más frecuente en las mujeres del mundo occidental. Un hecho muy importante es el cambio de presentación de los tumores mamarios en la actualidad. Además, cabe mencionar que las técnicas actuales de imagen presentan algunas limitaciones de diagnóstico, sobre-diagnóstico y un alto valor de falsos positivos. De esto se deduce la necesidad de buscar nuevas técnicas de imagen que complementen a las actuales y que resuelvan dichas limitaciones. La imagen funcional PET dedicada a mama (dbPET) es una técnica reciente que ha aportado resultados prometedores para afrontar el diagnóstico y el seguimiento de la patología mamaria desde un enfoque alternativo. En este proyecto proponemos desarrollar un estudio de la heterogeneidad tumoral mediante múltiples estudios dbPET y de su correlacción con diferentes parámetros clínico-biológicos. Para ello se propone estudiar: (i) el interés del dbPET en el diagnóstico de las lesiones mamarias cuando no es posible con las técnicas habituales; (ii) el papel del dbPET en los casos en los que se realiza la RM, con la finalidad de reducir el número de falsos positivos de esta última; (iii) la eficacia y valor prognóstico del dbPET en los casos de quimioterapia neoadyuvante y su interés en el seguimiento; (iv) la radiobiología de la braquiterapia mamaria, evidenciable a través de la imagen molecular y su efecto sobre el metabolismo de la glándula y (v) nuevos radiofármacos que permitan definir otras facetas biológicas del tumor y que puedan ser de interés clínico.
