They will be recruited in the province of Madrid (Hospital Universitario Ramón y Cajal, Fundación Hospital de Alcorcón, Hospital Universitario de Fuenlabrada, Hospital Universitario Príncipe de Asturias de Alcalá de Henares, Hospital Universitario La Paz-Carlos III, Centro de Salud Guayaba) and Barcelona (Unidad de Medicina Tropical y Salud Internacional Drassanes- Hospital Vall d’Hebrón). Each immigrant will be collected two stool samples and two nasal storundas: one of each in the first review and others after one year. The samples will be processed in the Microbiology Services of Ramón Hospital 1)- Detect and identify resistant bacteria (BLEE, carbapenemasas, VRE, MRSA) or high-risk clones present in the samples and 2)- Determine the composition of the microbiota by mass sequencing. The patient recruitment period will begin in January 2018 until December 2019. All subjects recruited should be seen a year after their inclusion in the study with a new stool sample and nasal swab. The third year of the project will be dedicated to the second visit of those recruited in the second year of the study and to finalise the molecular characterisation of the resistance mechanisms and strains that have been detected and the study of the microbiota by mass sequencing, as well as the analysis and dissemination of the data.     Objetivo principal. Detectar la colonización por bacterias resistentes a los antibióticos en inmigrantes recién llegados a nuestro país y analizar su evolución tras 12 meses de residencia. Búsqueda activa de clones de alto riesgo hospitalario. Metodología. Estudio observacional, prospectivo y multicéntrico. La población a estudiar serán los inmigrantes (n~100) recién llegados a nuestro país que lleven = de 4 meses en España. Se reclutarán en la provincia de Madrid (Hospital Universitario Ramón y Cajal, Fundación Hospital de Alcorcón, Hospital Universitario de Fuenlabrada, Hospital Universitario Príncipe de Asturias de Alcalá de Henares, Hospital Universitario La Paz-Carlos III, Centro de Salud Guayaba) y en Barcelona (Unidad de Medicina Tropical y Salud  Internacional Drassanes- Hospital Vall d´Hebrón). A cada inmigrante se le recogerán dos muestras de heces y dos torundas nasales: una de cada en la primera revisión y otras tras un año. Las muestras serán procesadas en los Servicios de Microbiología del Hospital Ramón 1)- Detectar e identificar bacterias resistentes (BLEE, carbapenemasas, VRE, MRSA) y/o clones de alto riesgo presentes en las muestras y 2)- Determinar la composición de la microbiota mediante secuenciación masiva. El periodo de reclutamiento de pacientes comenzará en enero de 2018 hasta diciembre de 2019. Todos los sujetos reclutados deberán ser vistos al año de su inclusión en el estudio con una nueva muestra de heces y torunda nasal. El tercer año del proyecto se dedicará a la segunda visita de aquellos reclutados en el segundo año del estudio y a finalizar la caracterización molecular de los mecanismos de resistencia y de las cepas que se hayan detectado y al estudio de la microbiota por secuenciación masiva, así como al análisis y divulgación de los datos.
