    Se pretende, mediante el uso de apps y warables, fomentar la activación de los pacientes para mejorar sus prácticas de autocuidado en personas con enfermedad crónica (cardiovasculares), mayores de 40 años con 2 o más fármacos diarios, donde se concentran los problemas de incumplimiento, errores de medicación del paciente y mayores dificultades para el control de factores de riesgo. Se diseña un estudio experimental (intervención aleatorizado, con 18 meses de seguimiento) con 2 grupos y medidas repetidas antes-después. Para el consenso de los planes estandarizados de autocuidado, según perfiles de pacientes, se aplicará la técnica de conferencia de consenso. La efectividad de la App se mide en términos de: grado de control de los factores de riesgo, autoeficacia en autocuidado, frecuencia de conductas saludables (ejercicio físico, dieta), cumplimiento terapéutico, uso seguro de la medicación, frecuentación (programada, demanda y urgente) y coste de la asistencia. It is intended, by using apps and wearables, promote activation of patients to improve their self-care practices in people with chronic disease (cardiovascular), over 40 years with 2 or more daily drugs, where compliance problems are concentrated , patient medication errors and more difficult to control risk factors. An experimental study (randomized intervention with 18 months follow-up) with 2 groups and repeated measures is designed with a also a before and after measure for each group. For consensus on standardized self-care, patient profiles according to plans, the consensus conference technique is applied. The effectiveness of the App is measured in terms of: degree of control of risk factors, self-efficacy in self-care, frequency of healthy behaviors (exercise, diet), treatment adherence, safe use of medication, frequency (scheduled, demand and urgent) and cost of care.
