| Property / summary: Predkladaný projekt má svojou podstatou a naplánovanými aktivitami vies k zavedeniu integrovaného modelu starostlivosti o zdravie a v konečnom dôsledku zvýi kvalitu a efektívnos poskytovaných zdravotníckych sluieb v predmetnom regióne, konkrétne v spádovej oblasti obce Krulov. Hlavnou podstatou projektu a teda aj jeho hlavným ciežom je prostredníctvom synergie medzi hlavným partnerom obcou a partnermi novovzniknutého Centra integrovanej zdravotnej starostlivosti (poskytovatežmi zdravotnej starostlivosti) zavies funkčný model kvalitného poskytovania zdravotných sluieb a to najmä za účelom zníenia nerovností z hžadiska prístupu ku kvalitným a efektívnym slubám veobecnej a pecializovanej ambulantnej zdravotnej starostlivosti a tie sociálnym slubám.Medzi pecifické ciele projektu môeme zaradi: zvýenie kvality zdravotníckej a sociálnej infratruktúry, zlepenie dostupnosti a prístupu k integrovaným zdravotníckym a sociálnym slubám, zníenie nerovnosti z hžadiska zdravotného postavenia, zníenie environmentálneho zaaenia. pecifické ciele reagujúce na hlavné problémy budú meratežné prostredníctvom týchto ukazovatežov: odhadované ročné zníenie emisií skleníkových plynov, počet renovovaných verejných budov, počet vytvorených centier integrovanej zdravotnej starostlivosti, počet zdravotníckych pracovníkov v etablovaných CIZS, podlahová plocha renovovaných verejných budov, zníenie ročnej spotreby primárnej energie vo verejných budovách, počet nástrojov zabezpečujúcich prístupnos pre osoby so zdravotným postihnutím a miera zabezpečenia bezbariérového prístupu osôb so zdravotným postihnutím k výsledkom projektu.Hlavný ciež a pecifické ciele projektu sa dosiahnu prostredníctvom týchto činností: vypracovanie kvalitnej projektovej dokumentácie, zabezpečenie transparentného procesu verejného obstarávania, výber dodávatežov (stavby, interiérového vybavenia, vybavenia zdravotníckej techniky, IKT vybavenia, komunikačnej techniky), modernizácia a rekontrukcia objektu Centra integrovanej zdravotnej starostlivosti vrátane jeho vybavenia interiérovým zariadením, zdravotníckou technikou, IKT vybavením a pod. pri zabezpečení úplnej debarierizácie a maximálneho zníenia environmentálneho zaaenia. Výsledkom implementácie projektu bude zavedenie funkčného komplexného manamentu starostlivosti o pacienta zaloeného na integrácii, ktorej základom je spolupráca lekárov primárneho kontaktu s lekármi ďalích pecializačných odborov, ktorí budú pôsobi v novozaloenom Centre integrovanej zdravotnej starostlivosti. | Property / summary: The present project, by its nature and planned activities, leads to the introduction of an integrated health care model and ultimately to increase the quality and efficiency of the health services provided in the region in question, namely the Krulov catchment area. The main thrust of the project and thus its main objective is through synergies between the main partner of the municipality and partners of the newly established Integrated Healthcare Centre (healthcare providers) to establish a functional model of quality provision of health services, especially in order to reduce inequalities from the demand for access to quality and effective general and specialised outpatient health care and social clubs. We can include: improving the quality of health and social infrastructure, improving accessibility and access to integrated health and social promises, reducing health inequalities, reducing environmental performance. Specific objectives responding to the main problems will be measured by means of the following indicators: the estimated annual greenhouse gas emission reductions, the number of refurbished public buildings, the number of integrated health care centres created, the number of health workers in the established CIZS, the floor area of the renovated public buildings, the reduction of the annual primary energy consumption in public buildings, the number of accessibility tools for persons with disabilities and the level of accessibility of persons with disabilities to the results of the project.The main objectives and specific objectives of the project shall be achieved through the following activities: development of quality project documentation, ensuring transparent process of public procurement, selection of suppliers (buildings, interior equipment, medical equipment, ICT equipment, communication technology), modernisation and reconstruction of the object of the Integrated Health Care Centre, including its equipment with indoor equipment, medical equipment, ICT equipment, etc. while ensuring complete debarierisation and maximum reduction of environmental performance. The result of the implementation of the project will be the establishment of a functional comprehensive patient care management based on integration, which is based on the cooperation of doctors of primary contact with doctors of other specialised disciplines who will operate in the newly established Centre of Integrated Health Care.
