Głównym celem projektu jest wzrost aktywności życiowej 12 osób (8K4M) potrzebujących wsparcia w codziennym funkcjonowaniu, zagrożonych ubóstw. lub wykl. społ., w tym osoby starsze, z niepełnosprawnością poprzez realizację usług opiekuńczo - aktywizujących i innych form wsparcia doraźnego świadczonych w DDP w Szostakowie w okresie trwania projektu. Usługi świadczone w DDP mają na celu pozostawienie uczestników projektu (UP) jak najdłużej w miejscu zamieszkania i mają stanowić istotne wsparcie w ich codziennym funkcjonowaniu. Usługi będą świadczone w formie usług świadczonych w lokalnej społeczność. Działalność placówki i opieka w niej świadczona aktywizuje osoby niesamodzielne społecznie i indywidualnie. Zakres działalności DDP jestszeroki, pomaga zorganizować czas w ciągu dnia, zaspokoić potrzeby towarzyskie i kulturalne, dbać o kondycję fiz i psych, zapewnia podstawową opiekę pielęgniarską. Oprócz towarzystwa innych osób, zapewnia zajęcia w różnych dziedz przy wykwalifikowanych trenerach. DDP zapewnia opiekę 8 godzin na dobę. Zad. w ramach projektu: 1.Funkcjonowanie DDP • Ind.wsparcie pracy socjalnej • Zatr. person spraw usługi opiekuńczo-społeczne • Zasiłki celowe i specjalne celowe • Wyżywienie UP • Transport UP • Zatrudnienie pielęgniarki 2. Warsztaty aktywizujące • Warsztaty związane z rozwijaniem zainteresowań oraz poznaniem możliwości spędzania czasu wolnego. • Warsztaty kształtowania kompetencji poznawczych i interpersonalnych • Warsztaty terapii ruchowej 3. Wsparcie specjalistyczne • Wsparcie psychologiczne • Wsparcie fizjoterapeuty Projekt wpisuje się w problemy, cele, przedsięwzięcia zidentyfikowane w LSR LGD „PB” oraz w ogłoszeniu o naborze wniosków. Wnioskodawca zapewnia trwałość miejsc świadczenia usług DDP utworzonych w ramach projektu po jego zakończeniu przez okres 2 lat. Realizacja projektu doprowadzi do zwiększenia liczby miejsc świadczenia usług opiekuńczych w społeczności lokalnej oraz liczby osób obj usługami w stosunku do roku 2019. (Polish)

The main goal of the project is to increase the life activity of 12 people (8K4M) who need support in their daily functioning, at risk of poverty. or bleed society, including the elderly, with disabilities through the implementation of care services – activation and other forms of emergency support provided at DDP in Szostaków during the project period. The services provided in the DDP aim to keep project participants at their place of residence for as long as possible and are intended to provide important support for their daily operation. The services will be provided in the form of services provided in the local community. The activity and care provided in the facility activates socially and individually dependent persons. The range of activities of DDP iswide, helps to organise time during the day, meet social and cultural needs, take care of physical and mental health, provides basic nursing care. In addition to the company of other people, it provides classes in various children with qualified trainers. DDP provides care 8 hours a day. As part of the project: 1.DDP operation • Ind. support for social work • Person for care and social services • Special purpose and special allowances • UP board • UP transport • Employment of a nurse 2. Activating workshops • Workshops related to developing interests and learning about leisure opportunities. • Workshops for shaping cognitive and interpersonal competences • Workshops of motor therapy 3. Specialist support • Psychological support • Support physiotherapist The project fits into problems, objectives, projects identified in the LSR LAG “PB” and in the vacancy notice. The applicant shall ensure the sustainability of the DDP service sites created under the project after its completion for a period of 2 years. The implementation of the project will lead to an increase in the number of places to provide care services in the local community and the number of people in service compared to 2019. (English)
