Sp.k., which aims to improve access to medical services, increase the effects of treatment and improve the quality of life of patients with chronic wounds. The idea behind the programme is to minimise hospitalisation of these patients and, by including telemedicine activities, to facilitate access to specialised care for mobilely limited patients. As part of the project, participants/patients will gain increased access to diagnostics and health services provided on an outpatient basis and to services provided in the patient’s environment (medical care, nurses, rehabilitation, psychologist). „Dostępne i efektywne leczenie w ramach podstawowej opieki zdrowotnej dla pacjentów z raną przewlekłą” to program realizowany przez SANTE CLINIC Sp. z o.o. Sp.k., którego celem jest poprawa dostępności do usług medycznych, zwiększenie efektów leczenia i poprawa jakości życia pacjentów obciążonych raną przewlekłą. Ideą programu jest minimalizowanie hospitalizacji tych pacjentów, a dzięki włączeniu działań z zakresu telemedycyny ułatwienie dostępu do opieki specjalistycznej pacjentom ograniczonym mobilnie. W ramach projektu uczestnicy/pacjenci uzyskają zwiększony dostęp do diagnostyki i usług zdrowotnych świadczonych w warunkach ambulatoryjnych oraz do usług świadczonych w środowisku pacjenta (opieka lekarza, pielęgniarki, rehabilitanta, psychologa).
