Proj (typy II i III) skier. do 70 osób (47K) zagrożonych ubóstwem i wykluczeniem społ., zamieszkałych w gminie Strumień. Celem proj jest wzrost samodzielności u min. 70% z 70 os potrzebujących wsparcia w codziennym funkcjonowaniu z Gminy Strumień (dalej: OPW, 47K), w tym 3 os z niepełnospr. (2K), poprzez m.in. mieszkanie wspomagane, usługi opiekuńcze i asystenckie świadczone w miejscu zamieszkania i Klub Seniora oraz zwiększenie kompetencji 20 opiekunów fakt.(15K) z zakr. opieki nad podopiecznymi w okr. 01.03.2021- 31.08.2022. Zadania w proj: wsparcie opiekuńcze, asystenckie, mieszkanie wspomagane, Klub Seniora, Centrum Wsparcia Opiek. Fakt. Rezultaty: 1. Liczba miejsc świadczenia usług społ. istniejących po zakończeniu proj: 59 (6 opiekunek, 1 AON, 50 miejsc w Klubie Seniora, 2 w mieszk. wspom.) 2. The aim of the project is to increase self-reliance in at least 70 % of 70 people in need of support in the daily functioning of the Municipality of Stream (hereinafter referred to as: GIP, 47K), of which 3 persons from part-spr. (2K), through, among others, assisted housing, care and assistance services provided at the place of residence and the Senior Club and the increase in the competence of 20 carers fact.(15K) from the care of children in the area. 01.03.2021-31.08.2022. Tasks in the project: care support, assistance, assisted flat, Senior Club, Care Support Center. That’s a fact. Results: 1. Number of service providers existing after the end of the project: 59 (6 babysitters, 1 AON, 50 seats in the Senior Club, 2 in the apartment.) 2.
