The project aims to reintegrate socio-inclusion. 70 persons (37K, 33M), residing in Hajnówka, Podlaskie Voivodeship, until 31.12.2021. It is a continuation of the project “Opportunities in integration”, implemented on 01.01.2017-30.09.2018 (completed and settled, indicators reached) and currently implemented “Opportunities in Integration II” (01.08.2018-31.01.2020). Target group: persons since 18 years experiencing multiple social exclusions due to more than 1 condition (description in IV.5). As part of the project, the project will be implemented: social work (with a social contract) and active social, professional and health integration. Support (min. 2 different forms for each UP) will be carried out on the basis of an individual path of social reintegration, created for UP with the help of a social worker, psychologist, mentors, incl. advisors, labour intermediaries and others based on diagnosis of the problem situation, resources, potential, predisposition and needs of UP. During recruitment, social workers, mentors, psychologists and job intermediaries will determine the diagnosis of candidates’ needs in relation to the activities offered in the project and the eligibility of candidates. Next, qualified UP will carry out activities according to the first part of the reintegration path defined by the social worker in the social contract and then the subsequent actions indicated by the substantive staff on the basis of predisposition and needs. Professional reintegration will not be implemented as the first element of support – it will take place at the earliest after the start of social work and the implementation of a minimum of 1h individual mentor support. The amount and forms of support are the result of a detailed analysis of the situation of people in the target group, while the scope and programme of social support meets the requirements and needs of the PLOs and employers. The result will be an increase in social competence in at least 34 % of UP and 25 % of UP or 12 % for people with disabilities, which will be the sustainability of the project. (English)

    Projekt ma na celu reintegrację społeczno-zaw. 70 osób (37K, 33M), zamieszkałych w Hajnówce w województwie podlaskim, do 31.12.2021. Jest kontynuacją projektu "Szansa w integracji", zrealizowanego 01.01.2017-30.09.2018 (zakończony i rozliczony, wskaźniki osiagnięte) oraz obecnie realizowanego "Szansa w integracji II" (01.08.2018-31.01.2020). Grupa docelowa: osoby od 18 r.ż. doświadczające wielokrotnego wykluczenia społ. z powodu więcej niż 1 przesłanki (opis w IV.5). W ramach proj. zostanie zrealizowana: praca socjalna (z kontraktem socj.) oraz aktywna integracja o charakterze społecznym, zawodowym i zdrowotnym. Wsparcie (min. 2 różne formy dla każdego UP) będzie realizowane na podstawie indywidualnej ścieżki reintegracji społ.-zaw., tworzonej dla UP przy pomocy pracownika socjalnego, psychologa, mentorów, doradców zaw., pośredników pracy i in. specjalistów na podstawie diagnozy sytuacji problemowej, zasobów, potencjału, predyspozycji i potrzeb UP. Podczas rekrutacji pracownicy socjalni, mentorzy, psycholog oraz pośrednicy pracy będą określać diagnozę potrzeb kandydatów w stosunku do oferowanych w projekcie działań oraz kwalifikowalność kandydatów. Następnie zakwalifikowani UP będą realizować działania wg I części ścieżki reintegr. społ. zaw. określonej przez pracownika socjalnego w kontrakcie socjalnym a następnie kolejne działania wskazane przez kadrę merytoryczną na podstawie predyspozycji i potrzeb. Reintegracja zawodowa nie będzie realizowana jako pierwszy element wsparcia -będzie się odbywać najwcześniej po rozpoczęciu pracy socjalnej oraz realizacji minimum 1h indywid. wsparcia mentora. Ilość i formy wsparcia wynikają ze szczegółowej analizy sytuacji osób z grupy docel., zaś zakres i program wsparcia społ.-zaw. odpowiada wymogom i potrzebomUP i pracodawców.
