The project responds to the perceived social problems: —the accelerated ageing process (increase in the number of single-person households of older people living alone with difficult access to health services) and the problem of increasing the economic burden on families, – the difficulty or inability to carry out basic life activities independently (as indicated by the Barthel scale) and the need for improvement measures for people with disabilities and dependent persons, especially at the age of 65+, – the small extent of securing the needs of older residents in terms of places of residence in daily homes, with a systematic increase in their number. As a result of the project, nursing and medical assistance will be stepped up as a continuation of hospital or outpatient treatment, including rehabilitation and physiotherapy, occupational therapy, and activities leading to improving and improving the well-being of these people. The project participants will be provided with free, comprehensive health services, transport to the place of health services, meals. (English)

    Przedmiotem projektu jest podjęcie działań skutkujących poprawą stanu zdrowia osób zagrożonych ubóstwem i wykluczeniem społecznym z gminy Korfantów i Łambinowice: osób starszych i niepełnosprawnych. W ramach proj będą świadczone usługi zdrowotne w dziennym ośrodki opieki medycznej dla osób starszych utworzonym w Opolskim Centrum Rehabilitacji. Uczestnicy projektu będą mieli możliwość korzystania z wypożyczalni sprzętu rehabilitacyjnego doposażonej w ramach zrealizowanego już projektu. W projekcie preferowane będą osoby ze wskazaniem lekarskim bezpośrednio lub do pół roku po odbytej hospitalizacji, spełniające warunki rekrutacji. W dziennej formie pobytu zostaną utworzone 4 miejsca opieki. Projekt stanowi odpowiedź na dostrzeżone problemy społeczne: -przyspieszony proces starzenia się ludności (wzrost liczby jednoosobowych gospodarstw domowych starszych osób samotnie mieszkających o utrudnionym dostępie do usług zdrowotnych) i problem związany ze wzrostem obciążeń ekonomicznych rodzin, - trudność lub niemożność samodzielnego wykonywania podstawowych czynności życiowych (wg wskazań skali Barthel) i konieczność podjęcia działań usprawniających u osób niepełnosprawnych, niesamodzielnych, szczególnie w wieku 65+, -niewielki zakres zabezpieczenia potrzeb starszych mieszkańców w zakresie miejsc pobytu w dziennych domach pomocy przy systematycznym wzroście ich liczby. W efekcie projektu zostanie zintensyfikowana pomoc pielęgniarsko-medyczna, stanowiąca kontynuację leczenia szpitalnego lub ambulatoryjnego, w tym rehabilitacja i fizjoterapia, terapia zajęciowa, będą realizowane działania prowadzące do usprawnienia i zwiększenia komfortu życia tych osób. Uczestnicy Projektu będą mieli zapewnione bezpłatne, kompleksowe usługi zdrowotne, transport do miejsca wykonywania usług zdrowotnych, wyżywienie. (Polish)
