Support under the project was identified following the diagnosis of needs carried out by W-ca in XI 2018. The following actions will be implemented under the project: — creation of 4 Senior Clubs for 70 people, offering care services 3 days a week/36 h/month/one Senior Club – the creation of a Daily Relief Centre for 15 people offering care services 5 days a week/8 h a day to seniors will be provided with infrastructure to spend their free time actively, as well as activating and engaging them in self-help activities for the local environment. Participants of the project will also be covered by telecare (so-called life bands) and will have the opportunity to participate in social and cultural life, including through participation in day trips. Outputs and results to be achieved in the project: — number of persons at risk of poverty or exclusion of the population covered by assistance and care services in the local community in the programme – 85 persons. — number of places created provide care services existing after the end of the project: The monthly cost of the participant’s support shall not exceed PLN 1 500. Celem gł. proj. jest zwiększenie dostępu do wysokiej jakości, przystępnych cenowo usług społecznych świadczonych w społeczności lokalnej w Gm. Rzekuń, w tym zwiększenie dostępu do usług opiekuńczych poza miejscem zamieszkania w formie Klubów Seniora – 70 os. i Dziennego Domu Pomocy - 15 os, w okresie od 01.11.2019 do 30.10.2021 r. W ramach proj. usługami opiek. objętych zostanie łącznie 85 os. (30 M) niesamodzielnych, starszych, zagrożonymi ubóstwem lub wyklucz. społecznym z terenu gm. Rzekuń. (woj. mazowieckie). Wsparcie w ramach proj. określone zostało na podst. przeprowadzonej przez W-cę diagnozy potrzeb w XI 2018 r. W ramach proj. realizowane będą następujące działania: - utworzenie 4 Klubów Seniora dla 70 os., oferujących usługi opiekuńcze 3 dni w tygodniu/36 h/miesiąc/jeden Klub Seniora - utworzenie Dziennego Domu Pomocy dla 15 osób, oferujących usługi opiekuńcze 5 dni w tygodniu/8 h dziennie Seniorom zostanie udostępniona infrastruktura pozwalająca na aktywne spędzanie czasu wolnego, a także zaktywizowanie i zaangażowanie ich w działania samopomocowe na rzecz środowiska lokalnego. Uczestnicy proj. objęci zostaną również teleopieką (tzw. opaski życia) i będą mieli możliwość uczestnictwa w życiu społeczno - kulturalnym, m.in dzięki udziałowi w wyjazdach jednodniowych. Gł. produkty i rezultaty, które zostaną osiągnięte w proj.: - licz. os. zagrożonych ubóstwem lub wykluczeniem społ. objętych usługami asyst. i opiek. świadcz. w społ. lokalnej w programie – 85 os. - licz. utworzonych miejsc świadcz. usług opiek. istniejących po zakończeniu proj.: 85 Śr. miesięczny koszt wsparcia uczestnika nie przekracza kwoty 1500zł.
