OBJECTIVE OF THE PROJECT: Increase the labour and social participation of 50 people (28Women) at risk of poverty or social exclusion who are not employed, including at least 25 people with disabilities from the communes of voivodships below the threshold of disadvantage (communes selected, point C1.7i) through comprehensive and individual support from 5.2020-4.2021Group:50 people(28 K) at risk of poverty or social exclusion living in municipalities see paragraph. 1.7 P is addressed only to persons from one or more of the following groups: — persons experiencing multiple exclusions, – persons with severe or moderate disability, – persons with associated disabilities and persons with mental disorders, including persons with intellectual disabilities and persons with general developmental disorders, RESULTATATES: —Number of people at risk of poverty or social exclusion who obtained qualifications or commies after leaving the programme:-45- Number of people at risk of poverty or exclusion of jobseekers after leaving:-10 – Number of people at risk of poverty or exclusion after leaving:13 1. Percentage of OzNs that made progress in social activation – 34 %, 2. Percentage of OzN that met the criterion of effectiveness of the problem – 12 %, 3. Share of remaining people at risk of poverty or social exclusion who have made progress in social activation -34 %, 4. Share of other people at risk of poverty or social exclusion who met the employment performance criterion – 25 %. Actions: 1. Diagnosis of individual needs and potentials of UP, 50 people x 3h 2. Activities in the field of specialist counselling and training of social skills,50 persons 3. Assisted employment trainer services, for 100 % UP 4. CEL PROJEKTU: Zwiększenie aktywności zawodowej i społecznej 50osób( 28Kobiet)zagrożonych ubóstwem lub wykluczeniem społecznym niezatrudnionych w tym min .25osób z niepełnosprawnościami z gmin woj.mazowieckiego znajdujących się poniżej progu defaworyzacji (gminy wybrane,pkt.C1.7i) poprzez kompleksowe i indywidualne wsparcie od 5.2020-4.2021Grupa :50osób(28 K) zagrożonych ubóstwem lub wykluczeniem społecznym zamieszkujących gminy patrz pkt. 1.7 P jest skierowany wyłącznie do osób z jednej lub kilku z niżej wymienionych grup: -osoby doświadczające wielokrotnego wykluczenia, - osoby o znacznym lub umiarkowanym stopniu niepełnosprawności, - osoby z niepełnosprawnością sprzężoną oraz osoby z zaburzeniami psychicznymi, w tym osoby z niepełnosprawnością intelektualną i osoby z całościowymi zaburzeniami rozwojowymi, REZULTATY: -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje lub kom po opuszczeniu programu:-45- Liczba osób zagrożonych ubóstwem lub wyklucz społ poszukujących pracy po opuszczeniu programu:-10 - Liczba osób zagrożonych ubóstwem lub wykluczeniem społ pracujących po opuszczeniu programu:13 1. Odsetek OzN, które dokonały postępu w aktywizacji społecznej – 34%, 2. Odsetek OzN, które spełniły kryterium efektywności zatrud – 12%, 3. Odsetek pozostałych osób zagrożonych ubóstwem lub wykluczeniem społecznym, które dokonały postępu w aktywizacji społecznej –34%, 4. Odsetek pozostałych osób zagrożonych ubóstwem lub wykluczeniem społecznym, które spełniły kryterium efektywności zatrudnieniowej – 25%. Działania: 1. Diagnoza indywidualnych potrzeb i potencjałów UP, 50 osób x 3h 2. Działania w zakresie poradnictwa specjalistycznego oraz trening umiejętności społecznych,50 osób 3. Usługi trenera zatrudnienia wspieranego , dla 100% UP 4.terapia psychologiczną/rodzinną /psychospołeczną lub program terapeutyczny dla osób uzależnionych - 20 UP 5.
