The objective is to increase the availability of health services for 150os. (85k and 65M) not dependent (ONS), including the elderly (min. 77. 65) from the woodland area in the period 04.2019-03.2022, through geriatric, care and rehabilitation services and the continuation of treatment in the context of the DDOM. The target group will be 150 os. from the area of forestry, whose state of health does not only allow POZ and AOS to be cared for and, at the same time, do not require round-the-clock surveillance of nursing and nursing care. It will be: . direct following hospitalisation, whose state of health requires increased nursing care, surveillance of pharmacological treatment, complex op. to facilitate and counter the progressive autonomy and support and education of health, as well as advice on the organisation of care and the treatment of autonomous care; Persons with a risk of hospitalisation in the near future or during a period of 12 months providing health services in the area of hospital treatment. The DDOM will provide care to a med. combined with a follow-up and improvement process in function and cognitive processes. DDOM support: Nursing care, including education for self-care and self care. . improving mobility. driver therapy. Supporting dogs. Necessary consultations. Food Adequate to health, dietary advice for dietary advice. preparing patient carers for continuation of care, making up among the 150 patients who have been taken care, at least 136 will have completed treatment, and 95 patients will improve quality and comfort. The implementation of the project will contribute to: A reduction in the number and duration of unjustified fresh hospitalisation on the part of the ONS. de-institutionalisation (English)

    Celem gł. proj. jest zwiększenie dostępności do usług zdrowotnych dla 150os. (85K i 65M) niesamodzielnych (ONS), w tym os. starszych (min. 77 os. pow. 65 r.ż.) z terenu pow. leskiego w okresie 04.2019-03.2022, poprzez świadczenia geriatryczne, pielęgnacyjne, opiekuńcze i rehabilitacyjne oraz kontynuację leczenia w ramach DDOM. Gr. docelową będzie stanowiło 150 os. z terenu pow. leskiego, których stan zdrowia nie pozwala na pozostawanie wyłącznie pod opieką POZ i AOS, a jednocześnie nie wymagają całodobowego nadzoru lek. i pielęgniarskiego realizowanego w trybie stacjonarnym. Będą to: . os. bezpośrednio po hospitalizacji, których stan zdrowia wymaga wzmożonej opieki pielęgniarskiej, nadzoru nad terapią farmakologiczną, kompleksowych dz. usprawniających i przeciwdziałających postępującej niesamodzielności oraz wsparcia i edukacji zdrowotnej, a także doradztwa w zakresie organizacji opieki i leczenia os. niesamodzielnej; . os., u których występuje ryzyko hospitalizowania w najbliższym czasie lub którym w okresie 12 m-cy udzielone zostały świadczenia zdrowotne z zakresu leczenia szpitalnego. DDOM będzie zapewniał opiekę med. połączoną z kontynuacją terapii i procesem usprawniania w zakresie funkcj. oraz procesów poznawczych. Wsparcie w ramach DDOM: . opieka pielęgn. w tym edukacja pacjenta dotycząca samoopieki i samopielęgnacji . doradztwo w doborze odpowiednich wyrobów med. . usprawnianie ruchowe . stymulacja procesów poznawczych . terapia zaj. . wsparcie psych. . niezbędne konsultacje lek. . wyżywienie adekwatne do stanu zdrowia . porady dietetyka . przygotowanie opiekunów pacjenta do kontynuacji opieki Dzięki proj. spośród 150 pacjentów, którzy zostali objęci opieką, min. 136 ukończy leczenie, a u 95 pacjentów polepszy się jakość i komfort życia.
