Sikorski 45, as well as the increase in the skills of 28 (16K and 12M) of real carers in the care of dependent persons until 31.12.2022 The project establishes a formal partnership between JST (Municipality of Malbork), PES (Higher School of Economy (Leader), which meets the def. PES). The project is addressed to people living with the KC in the Malbork district. The main tasks are: 1. The creation of 12 additional care facilities provided in a local community in the existing Farmhouse in Malbork, close to the center and the largest housing estate. For this purpose, the purchase of additional equipment and the full adaptation of the space to persons with disabilities are assumed. 2. Running a daily home of help for dependents is provided by standard and specialist care services selected in a comprehensive and individualised manner, based on the individual path of reintegration (ISR), taking into account the problems of potential, predisposition and the needs of the project participant. In newly created places, DDP will offer different forms of group and individual support, and a delivery service to and from DDP will be offered to dependants (WSG has a bus that will be used for this purpose). Support in the area not covered by the Healthcare Witness Act of 27.08.2004. 3. Provide support to real-life carers and people in the care environment to provide care services and improve their skills in caring for dependent persons. The project responds to the diagnosed problems and needs for the provision of social services, taking into account demographic trends, health, poverty and social exclusion and the level of accessibility of services in the area covered by the project. Cel projektu: Poprawa sytuacji życiowej 48(32K i 16M) osób niesamodzielnych poprzez zwiększenie dostępności do wysokiej jakości usług opiekuńczych świadczonych w społeczności lokalnej w istniejącej już placówce opieki dziennej Malborku, ul. Sikorskiego 45, a także wzrost umiejętności 28 (16K i 12M) opiekunów faktycznych w zakresie opieki nad osobami niesamodzielnymi do 31.12.2022 Projekt zakłada formalne partnerstwo JST (Gmina Miasto Malbork), PES (Wyższa Szkoła Gospodarki(Lider), które spełnia def. PES). Projekt adresowany jest do osób zamieszkujący zg. z KC na terenie powiatu malborskiego. Główne zadania to: 1. Stworzenie 12 dodatkowy miejsc usług opiekuńczych świadczonych w społ. lokalnej w istniejącym Domu Dziennej Pomocy w Malborku, znajdującym w pobliżu ścisłego centrum i przy największym osiedlu mieszkaniowym. W tym celu zakłada się zakup dodatkowego wyposażenia oraz pełne dostosowanie przestrzeni do osób niepełnosprawnych. 2. Prowadzenie dziennego domu pomocy dla osób niesamodzielnych świadcz. standardowe i specjalistycz. usługi opiekuńcze dobrane w sposób kompleksowy i zindywidualizowany, w oparciu o Indywidualną ścieżkę reintegracji (ISR), uwzględniającą problemy potencjał, predyspozycje i potrzeby uczestnika proj. W nowo powstałych miejsc DDP oferować będzie zróżnicowane formy wsparcia grupowego i indywidualnego, a osobom niesamodzielnym zostanie zaproponowana usługa dowozu do i z DDP (WSG dysponuje busem, który zostanie wykorzyst. tego celu). Wsparcie w zakresie nieobjętym ustawą z 27.08.2004 r. o świad. opieki zdrowotnej. 3. Zapewnienie wsparcia opiekunom faktycznym i osobom z otoczenia osób niesamodz. w pełnieniu usług opiekuńczych i zwiększenie ich umiejętności w zakresie opieki nad osobami niesamodz.Projekt odpowiada na zdiagnozowane problemy, i potrzeby w zakresie świadczenia usług społecznych, uwzględniając trendy demograficzne, stan zdrowia, poziom ubóstwa i wykluczenia społecz. oraz poziom dostępności usług na terenie objętym projektem.
