KC lives in the commune of Dziemiany in Pomorskie voivodship and are at risk of poverty and social exclusion due to the need for support in everyday operation, aimed at meeting the individual needs of the above-mentioned persons resulting from their dependence through the provision of care services and specialised care services provided in the place of residence, assistant services, Senior Club, rental of equipment supporting independence between 01.12.2020 and 31.11.2022. Proj responds to the problems and needs in the provision of social services identified in its real area, taking into account demographic trends and the level of availability of social services in the area. Target group: this is a wasp in need of support in the daily functioning of the commune of Dzemians. Accomplishment of tasks: —Assistance services in the place of residence -Special care services in the place of residence (rehabilitation, psychologist services) -assistance services -Guidance of the Senior Club -Lead of the rental of assistive equipment. Cel proj: zwiększenie dostępności do usług społecznych dla 50os potrzebujących wsparcia w codziennym funkcjonowaniu (30K+20M)-w tym 10os niepełnosprawnych, które w rozum. KC zamieszkują gminę Dziemiany w woj. pomorskim i są zagrożone ubóstwem i wykluczeniem społ. ze względu na potrzebę wsparcia w codziennym funkcjonowaniu, mających służyć zaspokojeniu indywidualnych potrzeb w/w osób wynikających z ich niesamodzielności poprzez realizację usług opiekuńczych i specjalistycznych usług opiekuńczych świadczonych w miejscu zamieszkania, usług asystenta, Klub Seniora, wypożyczalnię sprzętów wspomagających samodzielność w okresie od 01.12.2020 do 31.11.2022.
