Main objective of the project: Increase access to family support services in Moszczenica Municipality for 45 families, including 90 children by 18 years. 60Dz, 30Ch and 60 people from their families, parents of 45K, 15M, at risk of poverty or exclusion. Socially through the creation and operation of the Family Centre in the Commune of Moszczenica in the period 1 June 2020 31.05.2023.The target group of the project is 45 families at risk of poverty or exclusion. 60Dz,30Ch, which receive social assistance in accordance with paragraph of 12 March 2004 on social aid or eligible for social aid, i.e. satisfying at least one of the conditions laid down in Article 7 paragraph on social assistance or persons in need of support who have been diagnosed or suspected of having problems and social conflicts preventing the proper functioning of those persons in the environment whose planned activities carried out in the CR would make it possible to restore equilibrium from the area of the municipality of Moszczenica within the meaning of the provisions of the CC Malopolskie Province. Tasks:1. Establishment and operation of the Family Centre in Moszczenica;2. Diagnosis of the problem situation of UP together with the development of individual family assistance plans;3. Implementation of family support services at the Family Centre in Moszczenica in individual form;4. Implementation of family support services at the Family Centre in Moszczenica in group form;5. Cel główny projektu: Zwiększenie dostępu do usług wspierania rodziny na terenie Gminy Moszczenica dla 45 rodzin, w tym 90 dziecimłodzieży do 18 r. ż. 60Dz, 30Ch oraz 60 osób z ich rodzin rodzicówopiekunów 45K, 15M, zagrożonych ubóstwem lub wyklucz. społ. poprzez utworzenie i funkcjonowanie Centrum Rodziny w Gminie Moszczenica w okresie 01.06.2020 r. 31.05.2023 r.Grupę docelową proj. stanowi 45 rodzin zagrożonych ubóstwem lub wyklucz. społ., w tym 60 os. dorosłych 45K,15M i 90 dzieci młodzieży do 18 r.ż. 60Dz,30Ch, korzystających ze świadczeń pomocy społ. zgodnie z ust. z dnia 12.03.2004r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocy społ. tj. spełniające co najmniej jedną z przesłanek określonych w art. 7 ust. o pomocy społ. lub osobyrodziny potrzebujące wsparcia, u których zdiagnozowano bądź istnieje podejrzenie istnienia problemów i konfliktów natury społ. uniemożliwiających prawidłowe funkcjonowanie tych osóbrodzin w środowisku, którym przewidywane działania realizowane w CR umożliwiłyby przywrócenie równowagi, z obszaru gminy Moszczenica w rozumieniu przepisów KC woj. małopolskie.Zadania:1.
