Project implemented in the period 01.06.2020-31.07.2021 by OPOL REHABILITATION in partnership with Majchrzyk Piotr TRANS-MEDYK RATOWNICTWO. The project consists of the development of deinstitutionalised forms of medical care for the elderly, including those with disabilities, in the areas of, among others: support for long-term home medical care, provision of medical care as a substitute for actual caregivers, implementation at local level of solutions for coordinating the provision of medical services, upgrading the skills and qualifications of staff providing health services, increasing access to services by providing the necessary transport, and developing activities that improve the health of older people, including those with disabilities through motor rehabilitation, psychiatric rehabilitation, increased access to care, rehabilitation and support equipment through the creation of equipment rentals and psychological support and/or training for actual caregivers in medical care. This project is aimed at people or families at risk of poverty or social exclusion, the environment of people at risk of poverty or social exclusion and staff of services providing health services living in the voivodship of Poland, in particular the city of Opole and Krapkowice. Support will be provided to 8 patients(5K and 3M), 11(9K;2M) to staff of health services- 2 doctors[psychiatra – 1M and geriatra- 1K], 1 rehabilitator [1M],3 nurses [3K], 4 medical caregivers[4K], 1 psychologist [1K] and 5 actual caregivers (4K:1M). Projekt realizowany w okresie 01.06.2020-31.07.2021 przez OPOLSKIE STOWARZYSZENIE REHABILITACJI w partnerstwie z Majchrzyk Piotr TRANS-MEDYK RATOWNICTWO. Projekt polega na rozwoju zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami, w zakresie m.in.:wsparcia długoterminowej medycznej opieki domowej,zapewnienia opieki medycznej w zastępstwie za opiekunów faktycznych, wdrożenia na poziomie lokalnym rozwiązań z zakresu koordynacji realizacji usług medycznych, podnoszenia kompetencji i kwalifikacji personelu służb świadczących usługi zdrowotne, zwiększenia dostępu do usług poprzez zapewnienie niezbędnego transportu oraz rozwój działań wpływających na poprawę stanu zdrowia osób starszych, w tym z niepełnosprawnościami poprzez rehabilitację ruchową, psychiatryczna, zwiększenie dostępu do sprzętu pielęgnacyjnego, rehabilitacyjnego i wspomagającego poprzez tworzenie wypożyczalni sprzętu i wsparcie psychologiczne i/lub szkolenia dla opiekunów faktycznych w zakresie opieki medycznej. Niniejszy projekt skierowany jest do osób lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym, otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym oraz personelu służb świadczących usługi zdrowotne zamieszkujących teren woj.opolskiego w szczególności m.Opola oraz Krapkowic. Wsparciem zostanie objętych 8 pacjentów(5K i 3M), 11(9K;2M) osób stanowiących personel służb świadczących usługi zdrowotne- 2 lekarzy[psychiatra - 1M i geriatra- 1K], 1 rehabilitant [1M],3 pielęgniarki [3K], 4 opiekunów medycznych[4K], 1 psycholog [1K] oraz 5 opiekunów faktycznych (4K:1M).W ramach niniejszego projektu zostanie zapewnione świadczenie usług zdrowotnych nad osobami starszymi, w tym z niepełnosprawnościami.
