The project involves the creation in HOW in Smerzyna at the address of Smerzyn 62 Daily Residence for 60 people in need of support in the daily functioning of the municipality of Łabiszyn (in accordance with KC) and the implementation of care services for this group. The project will be implemented by the KUJAWSKO-Pomeranian flag of the Polish Scouting Association in partnership with the municipality of ŁABISZYN.The aim of the project is to increase access to social services for 60 people at risk of poverty or social exclusion, who need support in the daily functioning of residents (according to the KC) in the municipality of Łabiszyn (Kujawsko-Pomorskie Voivodeship) by 31.10.2022. Activities involving the creation and activity of DDP will provide support to those in need of day-to-day operation and activation according to the highest quality standards. The main tasks are:1. Creation of 15 places in DDP to provide social services (60 places in 4 editions of 15 people). To this end, adaptation works, the purchase of equipment and the full adaptation of the space to persons with disabilities (e.g. modernisation works) 2 are assumed. DDP providing care services tailored in a comprehensive and individualised manner to the needs of the participants (to the extent not covered by the Healthcare Act of 27 August 2004). The main result of the project będą:Liczba supported by the programme of places of provision of social services existing after the end of the project-15, The number of places created in the programme for the provision of assistance and care services existing after the end of project-15.After the end of the project, the durability of 15 places in the DDP will be maintained for at least a period corresponding to the duration of the project. Projekt zakłada utworzenie w HOW w Smerzynie pod adresem Smerzyn 62 Dziennego Domu Pobytu dla 60 osób potrzebujących wsparcia w codziennym funkcjonowaniu zamieszkujących (zgodnie z KC) Gminę Łabiszyn (4 edycje po 15 os.),oraz realizację dla tej grupy usług opiekuńczych. Projekt realizowany będzie przez CHORĄGIEW KUJAWSKO-POMORSKĄ ZWIĄZKU HARCERSTWA POLSKIEGO w partnerstwie z jst – GMINĄ ŁABISZYN.Celem proj. jest wzrost dostępu do usług społecznych dla 60 osób zagrożonych ubóstwem lub wykluczeniem społecznym, potrzebujących wsparcia w codziennym funkcjonowaniu zamieszkujących (wg.KC) na terenie Gminy Łabiszyn (województwo kujawsko-pomorskie) w terminie do 31.10.2022 r.Działania obejmujące utworzenie i działalność DDP zapewni osobom potrzebującym wsparcia w codziennym funkcjonowaniu dzienną opiekę i aktywizację wg najwyższych standardów jakościowych. Główne zadania to:1. Stworzenie 15 miejsc w DDP do realizacji usług społecznych (60 miejsca w 4 edycjach po 15 osób). W tym celu zakłada się prace adaptacyjne, zakup wyposażenia i pełne dostosowanie przestrzeni do osób niepełnosprawnych (m.in.prace modernizacyjne) 2. Prowadzenie DDP świadczącego usługi opiekuńcze dobrane w sposób kompleksowy i zindywidualizowany do potrzeb uczestników (w zakresie nieobjętym ustawą z dnia 27.08.2004 r. o świadczeniach opieki zdrow.). Głównymi rezult. proj. będą:Liczba wspartych w programie miejsc świadczenia usług społ. istniejących po zakończeniu projektu-15, Liczba utworzonych w programie miejsc świadczenia usług asys. i opiekuńczych istniejących po zakończeniu proj.-15.Po zakończeniu projektu zostanie zachowana trwałość 15 miejsc w DDP co najmniej przez okres odpowiadający okresowi realizacji projektu.
