The aim of the project is to develop and integrate the system of social services provided by Słupsk by creating the CUS from 01.10.2020-30.09.2023. Thanks to the project in Słupsk, a CUS will be created, thus creating new places of social services for families/disabled/actual caregivers. The planned date for the creation of CUS is 01.01.2021. In the structure of CUS there will be 3 teams: Social assistance team (implementation of social assistance tasks, including social work, social integration, social benefits), Family Benefits Team (family, St. educative, Alim. Fund) and Social Services Organisation Team (implementation of social services for all residents in the building). The target group of the project is CUS employees and residents of Słupsk, who will be able to benefit from the social services offered by CUS.CUS will seek to ensure coordination of social and health services and cooperation with public employment services through a formalised local partnership. This will facilitate both the acquisition of services from different sources and the management and coordination of service delivery processes. The partnerships will be made up of service providers. The tasks to be carried out under the project are: 1.Development of the CUS Implementation Plan, 2.The implementation and updating of the CUS Implementation Plan and the provision of social services for people with disabilities and actual carers; 3.The implementation and updating of the CUS Implementation Plan and the provision of social services for families 4.Elaboration of the Product Package. The duration of the project is 01.10.2020-30.09.2023. The project will result in the development of the CUS operating model and its pilot implementation. After completion of the project, Słupsk will promote the idea of creating CUS and accept study visits to present the solutions developed. Celem gł. projektu jest rozwój i integracja systemu usług społ. świadczonych przez gm. Słupsk poprzez utworzenie CUS w terminie 01.10.2020-30.09.2023. Dzięki projektowi w gm. Słupsk utworzone zostanie CUS, a tym samym powstaną nowe miejsca świadczenia usług społ. dla rodzin/osób niepełnosprawnych/opiekunów faktycznych. Planowany termin utworzenia CUS to 01.01.2021 r. W strukturze CUS funkcjonować będą 3 zespoły: Zespół ds. realizacji zadań z zakresu pomocy społecznej (realizacja zadań z zakresu pomocy społ., w tym praca socj., integracja społ., przyznawanie świadczeń pom. społecznej) ,Zespół ds.świadczeń rodzinnych (św. rodzinne, św. wychowawcze, fundusz alim.) iZespół ds. organizowania usług społecznych (realizacja usług społecznych dla wszystkich mieszk.w gm.). Grupę docelową projektu stanowią pracownicy CUS oraz mieszkańcy gm.Słupsk, którzy będą mogli skorzystać z usług społ. oferowanych przez CUS.CUS będzie dążyć do zapewnienia koordynacji usług społ. i zdrowotnych oraz współpracy z publicznymi służbami zatrudnienia, poprzez sformalizowane partnerstwo lokalne. Ułatwi to zarówno pozyskiwanie usług z różnych źródeł, jak i zarządzanie i koordynowanie procesami dostarczania usług. W skład partnerstw wchodzić będą dostarczyciele usług. Zadania, które zostaną zrealizowane w ramach projektu to: 1.Opracowanie Planu Wdrażania CUS, 2.Realizacja i aktualizacja Planu Wdrażania CUS oraz świadczenie usług społecznych dla osób z niepełnosprawnościami oraz opiekunów faktycznych; 3.Realizacja i aktualizacja Planu Wdrażania CUS oraz świadczenie usług społecznych dla rodzin 4.Wypracowanie Pakietu produktów. Okres trwania projektu to 01.10.2020 - 30.09.2023. Efektem projektu będzie wypracowanie modelu funkcjonowania CUS oraz jego pilotażowe wdrożenie. Po zakończeniu realizacji projektu gm.Słupsk będzie promować ideę tworzenia CUS oraz przyjmować wizyty studyjne w celu przedstawienia wypracowanych rozwiązań.Gmina Słupsk będzie realizowała projekt uwzględniając wszystkie 4 Kam.
