The proposal responds to the diagnosed problem of diabetes in Mazowieckie voivodship. The application is in line with the provisions of the Polish Government of Mazowieckie Voivodeship in the area of detection and prevention of diabetes. The aim of the project is to improve the health status of 2.700 people. 60r.ż (1.620K, 1.080M) from Radom and Zvolensk region by conducting screening tests for the detection of diabetes and educational activities in the field of diabetes – the goal will be achieved by XII.2021. The actions planned under the project, as described in Part D of the proposal, are: — Task 1: information Action (Module I), Task 2: medical consultations and educational activities (Module II) and Task 3: training of medical staff (Module III). The main group covered by direct support is the people of the region. 60 years of age living in Radom and the Zwolensk region, not yet diagnosed in the direction of diabetes, who will sign a print of informed consent to participate in the project. In addition, doctors/nurses, POZ from the above-mentioned area as “the surroundings”, will also be covered. The expected result indicator is the number of post-project health service delivery sites supported in the programme. The expected output indicators include: number of people at risk of poverty or social exclusion covered by health services in the programme. The project will meet the substantive criterion 1, 5, 6, 7, 8 – description in the various parts of the proposal. (English) | |||||||||||||||

Wniosek stanowi odpowiedź na zdiagnozowany problem związany z cukrzycą na ter. woj. mazowieckiego. Wniosek jest zgodny z zapisami RPZ Samorządu Województwa Mazowieckiego w obszarze wykrywania i profilaktyki cukrzycy. Celem projektu jest poprawa stanu zdrowia 2.700os. pow. 60r.ż (1.620K, 1.080M) z m. Radom oraz pow. zwoleńskiego poprzez przeprowadzenie badań przesiewowych w kierunku wykrywania cukrzycy oraz działań edukacyjnych z zakresu diabetologii – cel zostanie osiągnięty do XII.2021r. Zaplanowane w ramach projektu działania opisane w części D wniosku stanowią: - Zadanie nr 1: akcję informacyjną (moduł I), - Zadanie nr 2: konsultacje lekarskie i działania edukacyjne (moduł II) i - Zadanie nr 3: szkolenia personelu medycznego (moduł III). Podstawową grupą objętą wsparciem bezpośrednim są osoby pow. 60 r.ż. mieszkające w Radomiu i pow. zwoleńskim dotychczas niediagnozowane w kierunku cukrzycy, które podpiszą druk świadomej zgody na udział w projekcie. Ponadto działaniami zostaną objęci również lekarze/pielęgniarki, POZ z w/w terenu jako „otoczenie”. Zakładany wskaźnik rezultatu to liczba wspartych w programie miejsc świadczenia usług zdrowotnych, istniejących po zakończeniu projektu. Zakładane wskaźniki produktu to m.in.: liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie. Projekt spełni kryterium merytor. szczegółowe nr 1, 5, 6, 7, 8 - opis w poszczególnych częściach wniosku. (Polish)
