The “GO TO” project aims to increase the socio-professional activity and the employability of 180 people at risk of social exclusion in the city of Hajnówka as well as the environmental support for 60 children by 30.09.2022. as part of the envisaged actions, participants will be provided with comprehensive support through a reintegration pathway consistent with the individual diagnosis of the needs of each participant. The beneficiaries of the project will be those at risk of social exclusion, who will benefit from MOPs of which at least 13 % of the UP will be disabled persons (24UP). All UP will be covered by social contracts. The tasks to be carried out are as follows: 1.Social work module and financial support 2.Family support module, 3.Family support module 4. Unemployed document.5.Individual counselling module 6.Purchase of 9 persons with disabilities (in particular, with significant, moderate and coupled staff) for the transport of persons with disabilities.7.Environmental support module 8.Project management. The project is a direct response to the identified problems in the city, most notably unemployment/lack of work and high risk of poverty and progressive depopulation. The project will: • for people or those at risk of poverty or social exclusion, achieving the social efficiency index of at least 34 %, including an employment performance indicator of at least 25 %; • for persons with disabilities, reaching the social efficiency index of at least 34 %, including an employment performance indicator of at least 12 %. The project will improve the environment for family participants by improving the material status and the relationship between family members and other MOPs clients, thanks to the examples of “good practice” and the active approach to changing life.     Projekt"HEJ DO PRZODU”ma na celu wzrost aktywności społeczno-zawodowej oraz szans na zatrudnienie 180 osób zagrożonych wykluczeniem społecznym w mieście Hajnówka a także wsparcie środowiskowe 60 dzieci do 30.09.2022 r. W ramach przewidzianych działań uczestnicy zostaną objęci kompleksowym wsparciem w ramach ścieżki reintegracji, która będzie zgodna z indywidualną diagnozą potrzeb każdego z uczestników. Beneficjentami projektu będą osoby zagrożone wykluczeniem społecznym, które korzystają z pomocy MOPS w tym co najmniej 13% UP będą stanowiły osoby z niepełnosprawnością (24UP). Wszyscy UP zostaną objęci kontraktami socjalnymi. Przewidziane zadania: 1.Moduł pracy socjalnej i wsparcia finansowego 2.Moduł wsparcia rodziny, 3.Moduł wsparcia osób nieaktywnych zawodowo.4. Moduł wsparcia osób bezrobotnych.5.Moduł doradztwa indywidualnego.6.Zakup 9 osobowego busa przystosowanego do przewozu osób z niepełnosprawnością(w szczególności ze znaczną, umiarkowana i sprzężoną).7.Moduł wsparcia środowiskowego.8.Zarządzanie projektem. Projekt stanowi bezpośrednią odpowiedź na zdiagnozowane problemy w mieście, do których należą przede wszystkim bezrobocie/brak pracy oraz wysoki stopień zagrożenia ubóstwem i postępujące wyludnienie. Projekt zakłada: •w odniesieniu do osób lub środowisk zagrożonych ubóstwem lub wykluczeniem społ.–osiągnięcie wskaźnika efektywności społecznej na poziomie co najmniej 34%, w tym wskaźnika efektywności zatrudnieniowej na poziomie co najmniej 25%; • w odniesieniu do osób z niepełnosprawnościami-osiągnięcie wskaźnika efektywności społecznej na poziomie co najmniej 34%, w tym wskaźnika efektywności zatrudnieniowej na poziomie co najmniej 12%. Projekt korzystnie wpłynie na otoczenie uczestników projektu–rodziny poprzez poprawę statusu materialnego oraz relacji między członkami rodziny a także na innych klientów MOPS, dzięki przykładom „dobrych praktyk” i aktywnego podejścia do zmiany sytuacji życiowej.
