The project is aimed at people from rural territories experiencing multiple social exclusion, both unemployed, inactive, disabled and farmers and, depending on the diagnosis of the environmental needs of those at risk of poverty or social exclusion. The aim of the project is to combat social exclusion by activating social, health and professional social assistance workers. Each participant in the project will be identified by a professional integration professional, a psychologist and a professional adviser on the basis of needs and possibilities. Depending on the diagnosis, a participant will then be able to benefit from an active inclusion service of a social nature (individual and group meetings with a psychologist, the possibility to use the advice of a lawyer, a foundation for a computer under the guide — a digital exclusion specialist, a workshop of social competence and a workshop of parental competence (both in a stationary and outgoing form), a group of support based on the diagnosis of individual needs and of health services of a health nature (the possibility to use a physiotherapist to eliminate or reduce health barriers to the functioning or dispossession of the labour market). Project support according to the diagnosis will be tailored in a flexible way to each participant’s needs. In addition, participants will be able to benefit from an active inclusion service, where each person will be supported by a coacha, a professional adviser in individual and group meetings, and in accordance with the Individual Action Plan (IPD), will be able to benefit from training, work placements preceded by studies. Our comprehensive support is designed to improve participants’ life competences, to regain economic stability, to feel safe, to believe in their own strengths and capacities. (English)

    Projekt jest skierowany do osób z terenów wiejskich doświadczających wielokrotnego wykluczenia społecznego, zarówno osób bezrobotnych, nieaktywnych zawodowo, niepełnosprawnych, rolników oraz zależnie od diagnozy potrzeb otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym. Celem głównym projektu jest przeciwdziałanie wykluczeniu społecznemu poprzez aktywizację społeczną, zdrowotną i zawodową osób korzystających z pomocy społecznej. Każdy uczestnik projektu zostanie zdiagnozowany przez specjalistę do spraw ścieżki integracji, psychologa oraz doradcę zawodowego pod kątem potrzeb i możliwości. Następnie w zależności od diagnozy uczestnik będzie mógł skorzystać z Usług aktywnej integracji o charakterze społecznym (spotkania indywidualne i grupowe z psychologiem, możliwość skorzystania z porad prawnika, podstawy obsługi komputera pod okiem informatykiem - specjalista ds. wykluczenia cyfrowego, warsztatów kompetencji społecznych oraz warsztatów kompetencji rodzicielskich ( zarówno w formie stacjonarnej jak i wyjazdowej), grupy wsparcia uzależnionej od diagnozy indywidualnych potrzeb oraz Usług aktywnej integracji o charakterze zdrowotnym ( możliwość skorzystania z usług fizjoterapeuty w celu wyeliminowania lub złagodzenia barier zdrowotnych utrudniających funkcjonowanie lub powodujących oddalenie od rynku pracy). Wsparcie w ramach projektu zgodnie z przeprowadzoną diagnozą będzie dostosowane w sposób elastyczny do potrzeb każdego uczestnika. Ponadto uczestnicy będą mogli skorzystać z Usług aktywnej integracji zawodowej, gdzie każda osoba otrzyma wsparcie coacha, doradcy zawodowego na spotkaniach indywidualnych i grupowych oraz zgodnie z Indywidualnym Planem Działania ( IPD) będzie mogła skorzystać ze szkoleń, staży zawodowych poprzedzonych badaniami. Nasze kompleksowe wsparcie ma na celu podniesienie kompetencji życiowych uczestników, odzyskanie przez nich stabilizacji ekonomicznej, poczucia bezpieczeństwa, podniesienie wiary we własne siły i możliwości.
