Goal: Increase access to high-quality social services for 60 dependants and disabled people in Ciechanów by 31.12.2020. The recipients of the aid will be 60 people at risk of poverty and social exclusion. Support will be provided to independent persons, in particular the elderly and persons with disabilities, that require the provision of support to cover basic necessities of life and a treatment or rehabilitation process. Preference will be given to, inter alia, the persons and families covered by POFP, and the scope of support will not duplicate the activities carried out in this programme. The project involves providing community-based care for the inhabitants of Ciechanow. The project will be implemented in the partnership of the PCK OR in Ciechanów and the Municipality of Ciechanów, on behalf of which the Municipal Centre for Social Assistance will operate as a budgetary entity with experience in the provision of care services. There are no links between the partners referred to in Article 33 (3). 6 of the Act of 11 July 2014 on the rules for implementing the cohesion policy... The project envisages the implementation of such forms of support as: community-based care services and specialised services, providing and providing rental care equipment, counselling support to the family of non-samopouds, training for carers. The applicant shall ensure that the project leads to an increase in the number of persons supported and the number of places of provision of social services in relation to the data of the year preceding the submission of the application. Number of care facilities provided by the Applicant in 2016: 0. Number of sites carried out by the Applicant in addition to the project: 60. Support for the project will be based on the support path established individually for each participant and in accordance with the emulation concept. (English) | |||

Cel główny: Zwiększenie dostępu do wysokiej jakości usług społecznych dla 60 osób niesamodzielnych i niepełnosprawnych zamieszkałych w Ciechanowie do 31.12.2020r. Odbiorcami pomocy będzie 60 osób zagrożonych ubóstwem i wykluczeniem społecznym. Wsparciem objęte zostaną osoby niesamodzielne, w szczególności osoby starsze oraz osoby z niepełnosprawnościami, wymagające zapewnienia wsparcia w zaspokajaniu podstawowych potrzeb życiowych oraz procesie leczenia lub rehabilitacji. Preferowane będą m.in. osoby i rodziny objęte POPŻ, a zakres wsparcia nie będzie powielał działań realizowanych w tym programie. Projekt zakłada świadczenie opieki środowiskowej dla mieszkańców Ciechanowa. Projekt będzie realizowany w partnerstwie PCK OR w Ciechanowie i Gminy Miejskiej Ciechanów, w imieniu której na podstawie pełnomocnictwa działać będzie Miejski Ośrodek Pomocy Społecznej jako jednostka budżetowa posiadająca doświadczenie w świadczeniu środow. usług opiekuńczych. Pomiędzy partnerami brak jest powiązań, o których mowa w art. 33 ust. 6 ustawy z dnia 11 lipca 2014 r. o zasadach realizacji progr. w zakr. polityki spójności... Projekt zakłada realizację takich form wsparcia jak: środowiskowe usługi opiekuńcze i usługi specjalistyczne, doposażenie i prowadzenie wypożyczalni sprzętu pielęgnacyjnego, wsparcie doradcze dla rodzin osób niesamodz., szkolenia dla opiekunek. Wnioskodawca zapewnia, że projekt prowadzi do zwiększenia liczby osób objętych wsparciem oraz zwiększenia liczby miejsc świadczenia usług społecznych w stosunku do danych z roku poprzedzającego złożenie wniosku. Liczba miejsc świadczenia usług opiekuńczych przez Wnioskodawcę w 2016r.: 0. Liczba miejsc prowadzonych przez Wnioskodawcę dodatkowo w ramach projektu: 60. Wsparcie w projekcie będzie realizowane na podstawie ścieżki wsparcia stworzonej indywidualnie dla każdego uczestnika oraz zgodnie z koncepcją empowerment. (Polish)
