The project is aimed at dependent people, the residents of the Łódź Metropolitan Area who have found themselves in a group of people at risk of poverty or social exclusion as a result of chronic diseases and care, and are a response to the ageing of the population and increasing demand for support for dependent people and their carers. The project will look at nursing long-term care services provided at the patient’s home. This group often does not have sufficient financial resources to afford adequate care on its own. Low income, pensions, often abandonment of carers’ work cause a worsening of the raison d’être and need for social assistance. In contrast, the funds with the NFZ do not cover the needs of the target group to which the project is addressed. The limits, long queues, mean that many patients will never receive aid. The main objective of the project is to allow access for 80 dependants, in line with the principle of equal opportunities and non-discrimination, and equal opportunities for women and men, to health services as soon as possible, thereby contributing to the improvement of the quality of life of those persons and of their carers. The main task will be to provide care and assistance to self-employed people. Recruitment due to the expected high number of deaths will be continuous. A reserve list will be established. The promotion of the project will be to include information on the website, as well as in hospitals, clinics and social assistance centres. Due to the ban on the advertising of medicinal products, it is not possible to place notices in local media. (English) | |||

Powyższy projekt jest adresowany do osób niesamodzielnych, mieszkańców łódzkiego obszaru metropolitarnego, którzy w wyniku chorób przewlekłych i niesamodzielności znaleźli się w grupie osób zagrożonych ubóstwem lub wykluczeniem społecznym i jest odpowiedzią na starzenie się społeczeństwa i wzrastające zapotrzebowanie na wsparcie osób niesamodzielnych i ich opiekunów faktycznych. Przedmiotem projektu będzie pielęgniarska opieka długoterminowa świadczona w domu pacjenta. Grupa ta często nie dysponuje wystarczającymi środkami finansowymi, aby mogła pozwolić sobie na odpowiednią opiekę we własnym zakresie. Niskie dochody, emerytury, renty, często rezygnacja z pracy opiekunów powodują pogorszenie bytu i konieczność korzystania z pomocy społecznej. Natomiast fundusze z NFZ nie pokrywają potrzeb grupy docelowej do której skierowany jest projekt. Limity, długie kolejki powodują, że wielu pacjentów nigdy pomocy nie uzyska. Głównym celem projektu jest umożliwienie dostępu 80 osobom niesamodzielnym, zgodnie z zasadą równości szans i niedyskryminacji, oraz równości szans kobiet i mężczyzn, do usług zdrowotnych, w jak najszybszym czasie, co w rezultacie przyczyni się do poprawy jakości życia tych osób, oraz ich opiekunów faktycznych. Głównym zadaniem będzie zapewnienie opieki i pomocy pielęgniarskiej osobom niesamodzielnym. Rekrutacja ze względu na przewidywaną dużą liczbę zgonów będzie ciągła. Zostanie utworzona lista rezerwowa. Promocja projektu będzie polegała na umieszczeniu informacji na stronie www., oraz w szpitalach, przychodniach i ośrodkach pomocy społeczne. Ze względu na zakaz reklamy podmiotów leczniczych nie ma możliwości umieszczenia ogłoszeń w lokalnych mediach. (Polish)
