Main products and resolutions: (a) Number of people at risk of poverty or social exclusion covered by health services in the programme — 130b) Number of medicinal products covered by the provision of deinstitutionalisation tools — 13. ‘PROJECT’ means independent person — 86 K, 44 Mb) carers of dependent persons, in particular family members — 53 K, 27 M4. The tasks of the project are to support the deinstitutionalisation of care for dependent persons, by developing alternative care for dependent persons within the area of the Inowrocław sub-region (powiaty: Inowrocław, Mogileński, żniński). (a) Support for activities and the creation of new ininstitutionalised forms of care, including the 1 Daily Medical Care Service, in accordance with standards established in the framework of POWER (Standard DDOM adopted by the Steering Committee No 49/2017/XIV), (b) long-term med. domestic care of a non-taxable person, including nursing long-term care, (c) assistance to carers, in particular family members in the form of psychological support and training for dependants, (d) implementation at local level of arrangements for coordinating the delivery of services to independent persons. the project will be implemented in accordance with the Centre for the Coordination of Medical Care for dependent persons and provides access to the tender to be provided, i.e. DDOM or home-based care at the premises of each district falling within the subregional.5. Uchwałą nr 49/2017/XIV),b) długoterminową med. opiekę domową nad osobą niesamodzielną, w tym pielęgniarską opiekę długoterminową,c) pomoc dla opiekunów, w szczególności członków rodzin w postaci wsparcia psychologicznego i szkoleń w zakresie opieki med. nad osobami niesamodzielnymi,d) wdrożenie na poziomie lokalnym rozwiązań z zakresu koordynacji realizacji usług med. kierowanych do osób niesamodzielnych.Projekt będzie realizowany zgodnie z Modelem Centrum Koordynacji Opieki Medycznej dla osób niesamodzielnych i zapewnia dostęp do świadczonej oferty tj.
