Project implemented in the period 01.03.2019-31.12.2021. Setting up a Medical Care Coordination Centre for persons not alone., 1 DDOM, in the area of Tornia and 1 DDOM in m. Chełmno, provision of home-based care services combined with information and education support to dependent persons and their carers. The project is aimed at 540 persons, of which 324 K and 216 M resident within the meaning of CC, who are studying or working on the Ku-s region in the Toruń subregion. For the use of the model Centre for the Coordination of Medical Care for dependent persons, the implementation of support in the form of a Daily Care Service is in line with the DDOM standard developed in the framework of the POWER (B.1.5). The objective is to develop sustainable solutions for the high quality of health services, tailored to the individual needs of dependent persons. In addition, there will be a positive impact on the reduction in the number and duration of the unjustified medical hospitalisation of dependent persons, including the elderly. By improving access to healthcare services, which are used by persons who do not themselves, by reducing waiting time. In addition, it will improve the health of dependent persons and their carers. The project provides for comprehensive support for carers of people not themselves, including education and psychological support, which plays an enormous role in the recovery process. By means of such comprehensive support, which will be tailored to the needs of people who are not self-appointed and their carers can be taken up or continued by the carer. The health services provided in the framework will be provided at community level. In the framework of the Sub-Action (B.1.3). (English)

    Projekt realizowany w okresie 01.03.2019-31.12.2021 r. W ramach proj. objęto wsparciem subregion toruński tj. miasto Toruń, pow. toruński, pow. chełmiński, pow. golubsko-dobrzyński i pow. wąbrzeski(B.1.2). W ramach proj. zaplan. utworzenie Centrum Koordynacji Opieki Medycznej dla osób niesam., 1 DDOM na terenie m. Torunia i 1 DDOM na terenie m. Chełmno, świadczenie usług domowej opieki medycznej połączonych ze wsparciem informacyjno-edukacyjnym dla osób niesamodzielnych i ich opiekunów. Projekt skierowany jest do 540 osób, w tym 324 K i 216 M zamieszkujących w rozumieniu KC, uczących się lub pracujących na trenie woj. kuj-pom w subregionie toruńskim. Proj. będzie realizowany w oparciu o Model Centrum Koordynacji Opieki Medycznej dla osób niesamodzielnych, a realizacja wsparcia w formie Dziennego Domu Opieki Medycznej jest zgodna ze standardem DDOM wypracowanym w ramach POWER (B.1.5). Celem proj. jest wypracowanie trwałych rozwiązań zapewniających wysoką jakość usług medycznych, dostos. do indywidualnych potrzeb osób niesamodzielnych. Ponadto wpłynie pozytywnie na zmniejszenie liczby i czasu trwania nieuzasadnionych medycznie hospitalizacji osób niesamodzielnych, w tym starszych. Realizowany proj. zwiększy dostępność do świadczeń opieki zdrowotnej, z których korzystają osoby niesamodzielne, poprzez skrócenie czasu oczekiwania. Ponadto wpłynie pozytywnie na poprawę stanu zdrowia osób niesamodzielnych i ich opiekunów. Projekt zakłada kompleksowe wsparcie opiekunów osób niesam., w tym edukację oraz wsparcie psychologiczne, które odgrywa ogromną rolę w procesie zdrowienia. Dzięki tak kompleksowo zaplanowanej ścieżce wsparcia, która zostanie indywidualnie dostosowana do potrzeb osób niesam. oraz ich opiekunów możliwym będzie podjęcie lub kontyn. zatrudnienia przez opiekuna osoby niesam. Usługi zdrowotne świadczone w ramach proj. będą świadczone na poziomie społeczności lokalnych. Proj. jest zgodny z właśc. typami proj. przew. do dofin. w ramach poddziałania (B.1.3). (Polish)
