Pr. realiz. w ramach typu 2, skierow.do 60UP(32K) podopiecznych OPS w Radomiu zagroż.ubóstwem lub/i wyklucz. społ. oraz ich rodzin,zamiesz. na ter.pow. radomskiego (obszar defaworyz.), przy czym prefer.do obj. wsparciem będą os. lub rodziny korzystające z POPŻ, a zakr.wsparcia dla tych osób lub rodzin nie będzie powielał dział.,kt. dana os. lub rodzina otrzymała lub otrzymuje z PO PŻ. Zg. z DK. min. 50% UP stanonią os. z niepełnosprawnościami. W przypadku obj. wsparciem w ramach pr. os. bezrob.(50%UP) preferowane będą os. zakwalifikowane do III profilu pomocy zg. z ust. o prom.zatrudn. i inst. rynku pr. Celem pr. jest fakt.reintegracja społ.-eduk-zawod.60 UP poprzez objęcie ich 12 mc zindywidual. i kompleks. wsparciem wzmacn.opiekuńczo-wychow.funkcje rodziny oraz podnoszącym aktywn. społ-eduk-zawod. UP. Wszystk.przewidziane w pr. zadania mają na celu zwiększenie szans na zatrudnienie os. wykluczonych i zagroż.wyklucz. społ. oraz zapobieganie zjawisku wyklucz.społ. i ubóstwa. Wsparcie 60 UP obj. pr. odbędzie się na podst. ścieżki stworzonej indywid. dla każdego UP, z uwzględn. syt. prob.lub zagroż. syt. prob., zasobów, potencjału, predysp. i potrzeb- oprac. Indywid. Ścieżki Reintegracji. Każdy UP podpisze kontrakt socjalny lub dokument zawierający element. Analogiczne do kontraktu socjalnego. Pr. realiz. w partn. z właśc. teryt. OPS- OPS w Radomiu (partner), jednocześnie Wnioskodawca deklaruje współpracę w zakr.rekrut. UP z własc.teryt.PCPR lub PUP oraz zobowiązuje się do informowania tych podmiotów o udzielonych UP formach wsparcia. (Polish)
