CUS „WISIENKA” obejmuje swoim działaniem wszystkich beneficjentów, również tych gmin (dwóch) które do proj.nie przystąpiły. Celem proj. jest deinstytucjonalizacjia usług, oraz polepszenie dostępności miejsc świadczenia usługi , wzrost l.tych miejsc, zapobieganie odizolowania os.od rodziny i społeczności lokalnej, a gdy to nie jest możliwe, gwarancja tym os. warunków życia jak najbardziej zbliżonych do warunków domowych i rodzinnych oraz umożliwienie podtrzymywania więzi rodzinnych i sąsiedzkich. Gr.docelową proj. stanowią mieszkańcy pow.łódzkiego wschod. Wsparciem zostanie objęte łącznie 268 os.niesamodzielnych w podziale: Gm.Koluszki – 10 os (7K,3M), Gm.Andrespol – 40 os. (22K, 18M), oraz OT UP 40os. (21K, 19M) Gm.Nowosolna – 5M, Gm.Tuszyn - 6 os.(4K/2M) PCPR – 49 os. (27K, 22M) w tym 24 OT UP (12K, 12M) Stowa.Serc Jezusa i Maryi – 48 os. (25K. 23M) Stowa.Inicjatywa Rozsądnych Polaków – 70 os.(37K, 33M) Rezultaty; 1. L.wspartych w programie miejsc świad.usł.społ.istniejących po zakończenia proj.wynosi; 56 2. L.os.zagrożonych ubóstwem lub wykluczeniem społ.które opuściły opiekę instytucjonalną na rzecz usł.społ.świadczonych w społ.lokalnej w programie;7 3. L.utworzonych w programie miejsc świad.usł.asystenckich i opiekuńczych istniejących po zakończeniu proj.;52 4. L.os.zagr.ubóstwem/wykl.społ.poszu.pracy uczest.w kszt.szkole.zdobywających kwalif.po opusz.prog;11. Zadania; 1. Usługi Opiekuńcze w miejscu zamieszkania. 2. Wsparcie osób niepełnosprawnych i niesamodzielnych przez AOON. 3. Klub Seniora. 4. Dom Dziennego Pobytu. 5. Wypożyczania sprzętu rehabilitacyjnego. 6. Usługi wspierające dla opiekunów faktycznych specjalistyczne poradnictwo grupowe i indywidualne. 7. Rozwój specjalistycznych usług dla osób zależnych – posiłki (cały powiat). The CUS ‘hair’ covers all beneficiaries, including those municipalities (two) which have not joined the project. The aim of the project is to deinstitutionalise services, and to improve the availability of service places, to increase the number of places, to prevent the isolation of people from family and local communities, and, where this is not possible, to guarantee living conditions as close as possible to home and family conditions, and to enable family and neighbourhood ties to be maintained. The aim of the project is the inhabitants of the Lodz East. The support will cover a total of 268 non-reliable persons broken down by: Gm.Koluszki – 10 people (7K,3M), Gm.Andrespol – 40 people. (22K, 18M), and OT UP 40os. (21K, 19M) Gm.Nowosolna – 5M, Gm.Tuszyn – 6 pers.(4K/2M) PCPR – 49 people. L.supported by the programme of witnesses of the team after completion of the project, is carried out; IT’S 56 2. L.os. at risk of poverty or exclusion from the community who have left institutional care for the services provided in the local community in the programme;7 3. L. established in the program of witness places of assistance and care services existing after completion of the project;52 4. L.os.zagr.poverty/regular.społ.poszu.workparticipating in the form of a school. Tasks; 1. Care services in the place of residence. 2. Support for disabled and dependent people by AOON. 3. It’s the Senior’s club. 4. The House of Day Stay. 5. Renting rehabilitation equipment. 6. Support services for actual caregivers specialised group and individual counselling. 7.
