Cílem projektu je zlepšení kvality života lidí se závažnou duševní nemocí prostřednictvím efektivněji poskytované péče zahrnující propojení informací ze sociálního a zdravotního sektoru v rámci Terénního týmu (Práh jižní Morava) v JMK a zlepšení dostupnosti péče pro klienty v jejich přirozeném prostředí. Zároveň bude umožněn přístup k vybraným sdíleným datům i jiným profesionálům v oblasti péče o duševní zdraví, což přispěje k provázanosti potřebných prvků v systému a zlepšení koordinace péče. a.
