Celem proj. jest zaktywizowanie społeczno- zawodowe w okresie 1.06.2018-30.06.2019r.- 200os dorosłych (120K,80M) biernych zawodowo, zagrożonych ubóstwem lub wykluczeniem społecznym- będących os. korzystającymi ze świadczeń z pomocy społecznej zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocy społecznej- z tytułu ubóstwa (w tym min. 40 ON i doświadczających wykluczenia wielokrotnego) zamieszkujących w woj. wielkopolskim- poprzez realizację indywidualnej ścieżki reintegracji obejmującej instrumenty kompleksowej aktywizacji społeczno- zawodowej.Grupę docelową stanowi 200 (120K,80M) os biernych zawodowo, zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących zg z KC na obszarze woj. wielkopolskiego-będących os. korzystającymi ze świadczeń z pomocy społecznej zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocy społ.- z tytułu ubóstwa (tj. spełniające przesłankę ubóstwa zg. z art.7 ww.ustawy), w tym:- min.20% uczestników to os. spełniające przesłankę ubóstwa będące jednocześnie os. niepełnosprawnymi (40os; 24K,16M)- doświadczające tym samym wielokrotnego wykluczenia społecznego rozumianego jako wykluczenie z powodu więcej niż jednej przesłanki.W proj. realizowane będą instrumenty kompleksowej aktywizacji społeczno- zawodowej:- indywidualne spotkania z psychologiem i doradcą zawod. w celu utworzenia ścieżki reintegracji, IPD, podpisana zostanie umowa na wzór kontraktu socjalnego,- trening umiejętności społecznych,- indywidualne poradnictwo psychologiczne/ psychospołeczne- indywid. poradnictwo prawne,- indywid. szkolenia zawodowe,- pośrednictwo pracy,- staże zawod.W ramach projektu min. 90% uczestn. nabędzie umiejętności społ., a min. 80% kompetencje/ kwalifikacje zawodowe. Min. 34% uczestn. dokona postępu w procesie aktywizacji społecznej - zatrudnieniowej i zmniejszy dystans do zatrudnienia, a min. 22% podejmie zatrudnienie na min. 3 miesiące po zakończeniu udziału w proj.
