Projekt realizowany w latach 2017-2020 skierowany jest do co najmniej 385 rodzin biolog. i 70 dzieci z rodzin zastęp. (1600 os., w tym 927K i 673M) zagrożonych ubóstwem i wykluczeniem społecznym zgodnie z definicją zawartą w Wytycznych w zakresie realizacji przedsięwzięć w obszarze włączenia społecznego izwalczania ubóstwa z wykorzystaniem środków EFS, EFRR na lata 2014-2020 z terenu Gmin: Buk, Czerwonak, Kleszczewo, Luboń, Murowana Goślina,Oborniki, Pobiedziska, Puszczykowo, Rokietnica, Swarzędz, Szamotuły, Śrem, Tarnowo Podgórne oraz Powiatu Poznańskiego i Miasta Poznańmieszczących się na obszarze MOF Poznania oraz 60 os. (31K; 29M) prowadzących 50 rodzin zastęp. stanowiących otoczenie osób zagrożonych ubóstwemi wykluczeniem społecznym wskazanych powyżej.Zakres projektu:-utworzenie nowych miejsc świadczenia usług społecznych poprzez utworzenie 39 etatów dla asystentów rodziny;-utworzenie nowych miejsc świadczenia usług społecznych poprzez utworzenie 5 etatów dla koordynatorów rodzinnej pieczy zastępczej;-utworz nowych miejsc świadczenia usług społ poprzez utworzenie 1 etatu dla psycho oraz 1 etatu dla pedag oraz 1/2 et mediatora,-zaangażowanie 22 osób do pomocy przy sprawowaniu opieki na dziećmi i przy pracach gospodarskich w zawodowych rodzinach zastępczych;-przeprowadzenie 23 szkolenia (łącznie 94 h dydaktycznych) i 27 spotkań superwizji pracy dla rodzin zastępczych i pogotowia rodzinnego (60 os., w tym31K i 29M);-przeprowadzenie 27 033,5 h usług specjalisty dla osób zagrożonych ubóstwem i wykluczeniem społ zgodnie z definicją zawartą wWytycznych w zakresie realizacji przedsięwzięć w obszarze włączenia społecznego i zwalczania ubóstwa z wykorzystaniem środków EFS, EFRR na lata2014-2020 (w tym poradnictwo prawne, pedagog, psycholog, mediacyjne, psychiatr oraz terap. indyw., rodzin i par),-wzrost liczby rodziny wspierających na obszarze realizacji projektu (2 nowo powołane rodziny wspierające).Projekt realizuje wszystkie typy projektów ujęte w dz. 7.2.3 WRPO2014+.
