The aim of the project is to increase the number of places to provide care services by creating 18 places to provide environmental care services (non-stationary) in the city of Poznań and Wągrowiecki district. The project responds to the growing problem of aging residents and insufficient level of care support in the above-mentioned district. This objective will be achieved between 01.01.2018 and 31.12.2019 through 5 tasks: Task 1 and 2 Providing care services and specialised care services for dependants in their place of residence through the guardians of the partner or entities of the ESTask 3. Services supporting the work of carers and independent functioning of dependent persons. Task 4 and 5. Support of actual guardians in the form of 2 Careers Support Centres. Target group: The project is aimed at two groups living in the project: 120 dependent persons who will be covered by the temporary care services (at the place of residence of the person concerned). 50 actual caregivers (OFs) who will receive support raising their competences/caring skills.Main project indicators: Number of sites supported in the programme for the provision of environmental social services existing after the completion of the project – target value: 18. Celem projektu jest zwiększenie liczby miejsc świadczenia usług opiekuńczych poprzez utworzenie 18 miejsc świadczenia środowiskowych usług opiekuńczych (niestacjonarnych) na terenie miasta Poznania i powiatu wągrowieckiego. Projekt odpowiada na narastający problem starzenia się mieszkańców i niedostateczny poziom wsparcia opiekuńczego na terenie w/w powiatu. Powyższy cel zrealizowany zostanie w okresie 01.01.2018 r. - 31.12.2019 r. poprzez 5 zadań: Zadanie 1 i 2 Świadczenie usług opiekuńczych i specjalistycznych usług opiekuńczych dla osób niesamodzielnych w ich miejscu zamieszkania poprzez opiekunki partnera lub podmioty ESZadanie 3. Usługi wspierające pracę opiekunek i samodzielne funkcjonowania osób niesamodzielnych. Zadanie 4 i 5.
