The main objective of the project is to improve access to primary healthcare services and improve conditions for providing health services in Osiek City and Municipality by purchasing medical equipment with equipment and modernisation of the Osiek Health Centre. The project’s material scope includes: 1. Purchase of necessary medical equipment, including ultrasound, KTG, blood pressure recorder, heart rate detector (equipment to the Osiek Centre and Tursk Wielki-filia) 2. Carrying out modernisation works in the OZ building in Osiek (e.g. replenishment of defects in walls, painted). The implementation of the project will contribute, inter alia, to: — improving access to healthcare services in the month and the Osiek Municipality, – improving the quality of health care by raising the standard of medical services; — improving the level of health care infrastructure in the city and commune, – improving the health status of residents of Osiek City and Municipality thanks to the implementation of preventive programmes, and the model of comprehensive patient care – reduction of treatment costs which will result in improving the financial efficiency of the medicinal entity. — assistance for the return of the patient to active life in society and family; — equalisation of disparities between the level of access to health services in various regions of the voivodship The project assumes the implementation of coordinated models of care for the patient with cardiovascular diseases. MIEJSKO-GMINNY SAMODZIELNY PUBLICZNY ZAKŁAD OPIEKI ZDROWOTNEJ W OSIEKU

Głównym celem projektu jest poprawa dostępu do usług podstawowej opieki zdrowotnej oraz poprawa warunków udzielania świadczeń zdrowotnych na terenie Miasta i Gminy Osiek poprzez zakup sprzętu medycznego wraz z wyposażeniem oraz modernizację Ośrodka Zdrowia w Osieku. Zakres rzeczowy projektu obejmuje: 1.Zakup niezbędnego sprzętu medycznego m.in. aparat USG, KTG, rejestrator ciśnienia tętniczego, detektor tętna (sprzęt do Ośrodka w Osieku oraz Tursku Wielkim-filia) 2. Wykonanie prac modernizacyjnych w budynku OZ w Osieku (m.in. uzupełnienie ubytków w ścianach, malowane). Realizacja projektu przyczyni się m.in do: – poprawy dostępu do świadczeń opieki zdrowotnej w Miesicie i Gminie Osiek, – poprawy jakości opieki zdrowotnej w poprzez podniesienie standardu usług medycznych; – poprawy poziomu infrastruktury ochrony zdrowia na terenie miasta i gminy, – poprawy stanu zdrowia mieszkańców Miasta i Gminy Osiek m.in. dzięki wdrożeniu programów profilaktycznych, oraz modelu kompleksowej opieki nad pacjentem – zmniejszenie kosztów leczenia co będzie skutkować poprawą efektywności finansowej podmiotu leczniczego. – pomocy w zakresie powrotu chorego do aktywnego życia w społeczeństwie i rodzinie; – wyrównania dysproporcji pomiędzy poziomem dostępu do usług zdrowotnych w różnych regionach województwa Projekt zakłada wdrożenie skoordynowanych modeli opieki nad pacjentem ze schorzeniami układu krążenia.
