Celem projektu jest zwiększenie dostępności wysokich jakościowo usług zdrowotnych(w formie zdeinstytucjonalizowanej),zapewniające sprawność os.narażonych na wykluczenie społeczne.Dzięki temu zostaną im zapewnione-ochrona przed wykluczeniem oraz możliwość realnego i pełnego funkcjonowania w społeczeństwie.Grupę docelową stanowią osoby wykluczone/zagrożone ubóstwem/wykluczeniem społecznym oraz ich otoczenie.Założony cel zostanie osiągnięty poprzez przeprowadzenie kompleksowych procesów fizjoterapeutycznych,ukierunkowanych na zespoły bólowe kręgosłupa oraz prewencję upadków pacjentów geriatrycznych.Przewidywane wskaźniki: produktu:l.osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie:700os. rezultatu:l.wspartych w programie miejsc świadczenia usług zdrowotnych istniejących po zakończeniu projektu:5szt.
